Centerline Healthcare Partners, Inc- Clinical is Hiring a Part-Time/Full-time College Station/Bryan/Caldwell, TX - Nurse Practitioner Near Caldwell, TX

We have an exciting opportunity for Nurse Practitioner in the Bryan-College Station, TX area.

We are seeking part-time and full-time Nurse Practitioners to join Trumen Physicians and Associates. Responsibilities include:

  • Collaborating with the medical director in the management of patients
  • Meeting regularly with medical director to discuss patients and collaborate on the plan of care
  • Build and maintain a working relationship with facility administration
  • Effectively develop relationships with SNF Administrative and Clinical Teams
  • Act as a liaison between the SNF and Trumen Physicians
  • Assist and Track Patients in Trumen Value Based Programs

Requirements Include:

  • Must have a minimum of one year experience working as a Nurse Practitioner.
  • Willingness to work in Bryan-College Station
