Center for Life Management (CLM) provides Southern New Hampshire's most comprehensive array of outpatient mental health services for adults and elders. Our multi-disciplined treatment teams work with individuals ages 18 who experience severe and persistent mental illness and co-occurring disorders. We incorporate a variety of evidence-based practices in the treatment of our clients, one of which is Illness Management and Recovery (IMR). IMR supports individuals with reducing relapses, preventing re-hospitalization, managing distress from symptoms, using medications effectively, and achieving personal recovery goals. Coupled with functional support services, individuals are able to apply coping skills and techniques to reach their full potential. CLM is seeking an Adult Community Support Counselor to provide functional support and IMR services to help individuals improve the management of mental health symptoms, complete activities of daily living, adapt to life changes, and improve interpersonal functioning. This is a full time, 35 or 40 hour per week position.
Major Functional Areas and Description of Duties
  • Provide mental health counseling in the home and public settings as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan;
  • Develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement;
  • Educate natural supports and promote how to best encourage the client’s resiliency and recovery toward treatment goals;
  • Educate, problem solve and develop strategies to incorporate prescribed medication into daily routine;
  • Provide Illness Management and Recovery (IMR), as needed, to set goals, educate, promote hope, identify coping skills to cope with problems, symptoms and prevent relapse;
  • When providing IMR, complete outcome measures and adhere to the General Organization Index and Fidelity Scales to support program authenticity;
  • Provide crisis services, as needed, when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community;
  • Provide timely documentation on each service delivered;
  • Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care;
  • Provide set hours of billable service each week (eligible for incentive pay upon exceeding productivity expectation);
  • Serve on appropriate committees, both internal and external, as determined by supervisor;
  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients;
  • Execute all required services and documentation in accordance with agency policies and federal/state standards;
  • Maintain professional memberships, licenses, accreditation, certifications, etc.;
  • Represent CLM in a professional and appropriate manner in all settings;
  • Perform all other duties as assigned by supervisor.
Qualifications And Skills
  • Bachelor’s degree preferred in mental health field (social work, psychology, mental health counseling) however an Associate’s degree along with a minimum of two years of direct service may qualify;
  • Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations;
  • Proficient computer skills, specifically with all Microsoft office applications;
  • Ability to document effectively and efficiently in course of clinical work;
  • Ability to navigate an electronic health record;
  • Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision;
  • Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services;
  • Effective communication skills, organizational skills, and ability to multi-tasks.
Physical Demands and Working Conditions
A majority of the work will be visiting clients in the community and also in their homes. The ability to sit for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.
