Center for Human Development is Hiring a RN/LPN 2nd Shift Near Chicopee, MA

Seeking full time Registered Nurse/ LPN for evening or overnight shifts in a team environment providing care to adults and/or teens at CHD's Crisis Stabilization; this is part of a single location providing crisis, outpatient behavioral health and brief stabilization care. The right candidate will have a sense of humor, be self-motivated and a team player with a harm-reduction orientation to care. Shift differential and Sign-On Bonus available.

Essential duties for these roles may include:

  • Collaborating with nurses, physicians, advance practice nurses, psychologists, social workers, or other professionals
  • Collaborating with guardians, relatives, and significant others of people served when clinically indicated
  • Documenting assessments and treatments in the electronic health record
  • Monitoring vital signs
  • Assessment of signs of substance withdrawal and completion of standardized assessment tools such as the Clinical Opioid Withdrawal Scale (COWS, CIWA, CIWAb)
  • Fills physician orders
  • Administers medications
  • Engages in a medication reconciliation process
  • Coordinates medical care
  • Contributes to comprehensive assessments
  • Brief crisis counseling
  • Individualized crisis prevention planning
  • Providing psychoeducation
  • Assisting with discharge planning and care coordination.
