Part time Job (20 hours per week)

Neighborhood Health Ambassador

$23.75 per hour

Background:

The Center for Human Development is a community-based organization that offers a spectrum of services for at-risk youth, individuals, families, and communities in Contra Costa County. Since 1972, we provide services aimed at promoting growth, supporting resiliency, and empowering people. Our dedicated staff and several hundred volunteers create and deliver programs and services addressing wellness and prevention, youth leadership, conflict resolution, parenting skills, and other challenges facing the community.

Center for Human Development is facilitating of the HUB Operations and Expansion of the Lazarex Cancer Foundation Cancer Wellness Program through a grant agreement.As experts in their neighborhoods, the Neighborhood Health Ambassadors will serve as a liaison between the neighborhoods in East Contra Costa County for the Lazarex Cancer Foundation.

Job Description:

  • Neighborhood Health Ambassadors engage, recruit, support, advocate, and educate community members about cancer, cancer screening, cancer treatment, and clinical trials.
  • Neighborhood Health Ambassadors conduct community outreach, connect community members to services and resources as needed, advocate for community member’s health needs, and identify and surmount barriers to access cancer screening, treatment, and clinical trials.

Principal Duties:

  • Implements outreach strategies to identify places for health education conversations at sites, such as barbershops, laundromats, nail and hair salons, senior centers, faith-based organizations, and community centers in their neighborhoods.
  • Engages and connects community members to the Cancer Wellness HUB and Cancer Care Companion for cancer awareness and risk reduction programs and referrals to screenings treatment and clinical trials information.
  • Provides cancer risk reduction information to individuals and groups using specially designed health educational materials.
  • Assists with the navigation, coordination, and connection of community members to services and programs with the Cancer Care Companion at the Cancer Wellness HUB.
  • Makes referrals to cancer screening and treatment sites, cancer survivor/caregiving support groups, and clinical trials.
  • Completes a Hub Interest Form electronically, documents all community encounters, maintains comprehensive electronic contact files, helps schedule appointments, and submits reports to the Cancer Care Companion.
  • Maintains contact with community members and follow up with them on their scheduled screenings, treatments, and clinical trials to ensure needs are being met.
  • Identifies barriers to accessing screenings and clinical care and works with the hub team to create strategies intended to reduce barriers and accompany community members to appointments if needed.
  • Attends regular team meetings, and performs other work-related duties as requested.

Knowledge, Skills, and Abilities:

  • Excellent interpersonal skills with the ability to communicate with individuals from diverse backgrounds.
  • Commitment and passion to work for reducing health disparities in the African American and immigrant communities.
  • A dynamic team player able to work collaboratively and effectively with fellow Neighborhood Health Ambassadors and other team members.
  • Familiarity with the neighborhoods in East Contra Costa County, including familiarity with community-based organizations, senior and recreation centers, faith-based organizations, and/or service clubs.
  • Self-starter, able to set daily and weekly objectives and work independently in the field.
  • Ability to multitask while constantly meeting deadlines.
  • Writing skills, including word usage and spelling, rules of composition.

Education, Credentials and Experience:

  • Minimum of 1 year of work experience
  • Associate or bachelor’s degree is desirable.
  • Experience conducting community outreach and organizing group meetings is highly desirable.
  • Personal experience living with cancer is a plus.
  • Knowledge of Microsoft Office software, Google Suite, and Zoom
  • Familiarity with data entry into cloud-based systems is helpful.

  • Occasional evenings and weekends as requested
  • Valid California Driver’s License (Required)
  • Reliable transportation and proof of insurance (Required)
  • A successful background check (Required)
  • Clear T.B. test. (Required)

Reports to:

CHD Program Manager

Salary:

Fee rate: $23.75 hourly rate. This is a part time job (20 hours per week), non-exempt position.

Benefits:

Benefits include medical, dental, vision, 12 holidays and 19 vacation days per year and a retirement plan.
