Position Summary:

Blended Case Management is an outreach service assisting individuals of all ages and their families with assessment and service planning, linking with services and accessing community resources, problem resolution, identifying strengths, working towards identified goals and mental health recovery.

Essential Functions:

• The Blended Case Manager (BCM) provides case management services to adults, adolescents, and children who suffer from a serious mental illness or emotional disturbance. The BCM works with the client and families to access, link, coordinate and monitor needed services as well as to provide support and advocacy to assist clients in maintaining in their home or community; contact is frequent and intensive.
• Maintain a caseload of 30 individuals.
• Develop service goals based upon strengths of the individual.
• Complete all reports and documentation in a timely manner according to quality standards.
• Work a flexible schedule and participates in an On-Call Rotation to ensure 24-hour coverage for holidays, weekends and daily, between the hours of 4:30 pm-8:30 am.
• Able to manage time effectively to meet the needs of consumers and program required service unit expectations.
• Have excellent verbal and written communication skills, computer competency.
• Other duties as assigned to achieve the mission of the agency.

BCMs are available to meet with consumers during normal business hours, evenings and on weekends when necessary to meet the needs of the individual or when an individual is experiencing a crisis event. Blended Case Management has a crisis component to the service delivery and all BCM’s are scheduled a rotation of on call.

BCM’s receive:

· University of Pittsburgh/WPIC BCM Training and education

· Regular individual and group supervision

· Company computer and cell phone

· Mileage reimbursement is available at the state set rate

The starting rate is $18.50/hour but may start higher depending on experience and education. We pay a stipend of $250.00 per week when covering on call for one full week.

Qualifications:

Bachelor’s degree in Human Services or related field. One year experience in MH Case Management or in the mental health field preferred. Knowledge of case management principles, computer literate, familiarity with the Recovery Model, and knowledge of Northumberland County resources.

Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances will be required. Individuals who have accessed human services are encouraged to apply. EOE/ADA
