Community Service Supervisor

Reports to: Director of Community Integration

We are a local non-profit organization founded in 1970 by Louisville-area parents of people with IDD. What you’ll do:

The Community Service Supervisor will assist in monitoring and service provision in Cedar Lake Residences (CLR) independent and supported living programs. This role will provide supervision to Direct Support Staff (DSP) and Community Integration Specialists (CIS) which entail monitoring productivity standards and assessing caseload utilization in the most effective and efficient manner. This position will function as a member of the CLR team and may provide supplementary assistance to any individual served by CLR.

  • Assists in ensuring regulatory compliance by maintaining appropriate records related to state regulations and CLR policies. Assists management in the maintenance of the electronic record by uploading and generating reports.
  • Ensures staff needs are met and issues are addressed in a timely, efficient, and confidential manner. Updates the Director of DSP/CIS concerns daily.
  • Completes written evaluations of DSP/CIS within 60 days following employment, and annually thereafter.
  • Maintains documentation according to specific waiver requirements, which includes a full description of services rendered that entails a summary reflecting progress, outcomes, dates of services, implementation, goal projection and barriers/concerns with services. Helps with preparation of paperwork needed for HUD and Medicaid within specified timeframes.
  • Assists with facilitating and coordinating individual benefits with the individuals we support to encourage self-advocacy and comprehension related to Social Security, Medicaid and Waiver services, medical appointments, and pharmaceutical needs. Works collaboratively with the Benefits Coordinator.
  • Assists in the provision of budgeted support hours at designated property to ensure optimal guidance. Provides support and assistance regarding prior authorization utilization within community services such as: community access (CA), community living supports (CLS), homemaker services (HM), personal assistance (PA), and supported employment (SE).
  • Participates in Plan of Care (POC) meetings and assessments, with SCL/MPW team members or outside agency support staff, as appropriate. Encourages implementation of goals with the objective of increasing self-reliance and independence. Ensures that required service documentation reflects progress and concerns.
  • Maintains production standard of 50% billable time, as directed by Director.
  • Encourages and assists individuals in exercising their rights while in community settings. Facilitates constructive ways to respond to conflict and develop strategies for resolution. Provides guidance and direction on social/judgement and health related issues that pertain to integration in the community.
  • Attends in-services, staff trainings and meetings, as scheduled.
  • Represents CLR in a professional manner providing ethical standards of practice for individuals we support.
  • Shares on-call responsibilities
  • Performs other duties as assigned.

What sets you apart:

  • Degree in human services field and/or QIDP/QDDP status is preferred, but not required.
  • High School Diploma/GED is required.
  • Valid Kentucky driver’s license & auto insurance.
  • 1 year of related experience working with individuals with IDD is preferred.
  • First aid and CPR certified.

What your typical schedule looks like:

  • Full-time, Monday-Friday, hourly. On-call responsibilities. May include occasional weekends and evenings to meet individuals’ needs.
