Overview

Full-time (37.5 hrs. per week, Mon.-Fri.) – Starts at $23.55/hr. - $24.73/hr. (D.O.E/Education), with COMPETITIVE BENEFITS INCLUDING:

Per the current CBA, union positions will be subject to a 3% increase 7.1.24 and a 7% increase 7.1.25.

Compensation is set by agreed to language in the Collective Bargaining Agreement between CCSNW & SEIU 1199NW and determined based on experience.

The CCS NW Recovery Centers provide a full continuum of outpatient chemical dependency treatment services. CCS provides treatment to youth, adults, and their families. The CCS NW Recovery Centers are a safe, nurturing, and enriching environment where all are treated with dignity and respect, and where clients and their families become empowered to make healthy changes to stop the destruction of alcoholism and other drug dependencies. CCS emphasizes a broad range of approaches to the treatment of alcoholism and other drug dependencies, striving to serve those most in need. The philosophy of CCS NW Recovery Centers is that treatment is most effective when it is “family-centered”.

The Case Support Specialist is responsible for providing direct service case support to Substance Use Disorder clients and their families in accessing treatment and other needed services including but not limited to access to telehealth and inpatient treatment referrals. The Case Support Specialist is also responsible for providing overall support to the Clinical Team including but not limited to case support for engagement and telehealth services including administrative support.

Responsibilities

MAJOR DUTIES AND RESPONSIBILITIES:

CASE SUPPORT/ENGAGEMENT SERVICES:

  • Ensure that reminder and engagement calls for initial, ongoing and missed appointments are made in a timely manner.
  • Work with clients to remove barriers to attending treatment such as transportation and daycare.
  • Engage with new clients to see how treatment is going for them. Offer support and problem solve around any issues they may have to keep them engaged.
  • Follow up to individuals who have dropped out of services to motivate them to return. Problem solves any issues that are keeping them from attending in-person or accessing telehealth services.
  • Responsible for coordinating residential inpatient treatment for clients and facilitate discharge back to outpatient treatment to ensure they re-engage successfully and timely.
  • Responsible for assisting clients in accessing substance abuse disorder treatment.
  • Responsible for ensuring clients and their families have access to optional treatment funding including publicly funded resources as appropriate.
  • Responsible for assisting clients in accessing community resources such as food banks, shelter, transportation, medical care, TB testing, employment and training/job search, etc.
  • Assist with urinalysis collection as needed.
  • Responsible for monitoring time between individual sessions, Treatment Plan reviews and last dates of service over 30 days for each client to inform clinicians of due dates.
  • Communicate all engagement work done with clients to their primary SUD clinical.

TELEHEALTH SUPPORT:

  • Responsible for follow up with clients engaged with telehealth services to ensure they are able to access their assigned treatment groups through Teams.
  • Responsible for providing assistance and instruction to clients with connecting to Teams for treatment services where needed. 
  • Responsible for following up with clients who have not engaged in telehealth services to problem solve any barriers to connecting to services.

ADMINISTRATIVE SUPPORT:

  • Assist SUDP/SUDPT with client record management:
    • Maintain accurate and timely records on each client according to the Agency’s client record policies and procedures.
    • Input and maintain accurate and timely data within the EHR record system.
    • Prepare and submit timely reports to internal billing department and other entities as requested.
    • Responsible for coordinating with staff and other agencies and organizations to expedite treatment for each client and their continuum of care including scheduling residential bed dates and forwarding required documents.
    • Attend staff meetings and training as required.
    • Undertake special projects and job assignments as requested by the supervisor and/or Lead SUDP.
    • Work collaboratively and respectfully with staff in all disciplines to develop continuously improving client services.

ACCESS TO AND PROTECTION OF PHI:

  • Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area).
  • Complies with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms).

Other duties as assigned.

Qualifications

MINIMUM:

  • 6 months experience working in the SUD (substance use disorders) or medical field(s) OR medical related educational certificate.
  • Experience working with challenging populations in a fast-paced environment.
  • Requires the ability to establish and maintain effective working relationships with staff, Community partners and professionals.
  • Computer experience including data entry.
  • Excellent communication skills (both verbal and written).
  • Requires time management and interpersonal skills.
  • Ability to work both independently and as a member of a team.

PREFERRED:

  • Case Management experience.
