Overview

Program Description

Case Manager will provide case management to identified units as part of a Permanent Supportive Housing program at Sunset Gardens Apartments in Renton, WA.

Position Description

Coordinate screening, intake and case management service at the Sunset Court and Cedar Park Apartments in Renton of at least 20 households. This position’s work station is off-site from the CCS Kent Family Center office.

Responsibilities

Provide case management for the individuals that reside in the identified units at Sunset Gardens Apartments.

• Work with clients to develop individual goal plans. Work with individuals to monitor progress, provide advocacy-based information and referral, including life skills, budgeting, jobs/education, crisis invention and mental health.• Ensure accuracy and maintenance of all client records, including service notes and outcomes; prepare reports as indicated by contracts.• Submit monthly reports to contractors and supervisor.• Interview, screen, and select qualified individuals for the program when vacancies arise in units.• Provide transportation for clients as needed/appropriate.• Work with outside community providers to incorporate more services with the program.• Ensure that Incident reports are written and given to the Division Director for review.• Assist clients with completing housing applications, and maintain relationship with housing authorities to assist clients in securing other permanent housing as needed.• Collaborate with service providers to work with clients at Sunset Gardens Apartments.

Promote the CCS Mission in the community.

• Network with other social service agencies to advocate for the program and clients; be available to educate and advocate in the community.• Treat clients in a culturally sensitive manner and actively seek to become informed on issues where culturally-determined perceptions may be significant.

Supervise On-Call staff for interpreter purposes in case management of individuals, as needed.

• Coordinate appointments with individuals and On-Call staff to provide interpreter services.• Monitor On-Call staff to ensure hours do not exceed full-time status.• Train staff on case management issues.

Administrative and General Responsibilities.

• Attend meetings related to this contract.• Attend staff meetings, workshops, retreats, and in-service classes as scheduled.• Maintain accurate record of hours worked and turn in timesheets on schedule.• Complete all information into data management system on a timely basis. Work in conjunction with agency data management staff to ensure accuracy and completeness.• Contribute to and support a positive, team-oriented work environment; participate with other staff members in group decision-making process. • Maintain cleanliness and order of work area. • Perform other job-related duties as assigned.

Job Conditions

This position requires the employee to work in an environment where there may be exposure to blood, bodily fluids, and other potentially infectious material. Other exposures could include cleaning supplies, bedbugs, chemicals involved in pest control, paint and other materials used in building maintenance, building temperature fluctuations, dust, noise, and odors. Working conditions include interruptions, crisis response, and regular interactions with individuals who are dealing with issues of chemical dependency and/or mental illness, or are experiencing anger or frustration.

Qualifications

  • BA in Social Work or related field, or equivalent experience.
  • One year working with high-risk or homeless populations.
  • Ability to work both as a member of a team and independently.
  • Reliable transportation, driver’s license, insurance and driving record that that meets CCS Employee Driving Policy requirement for driving agency vehicle and/or transporting clients.
  • Experience with crisis intervention.
  • Ability to work both independently and as a member of a team.
  • Ability to supervise others including hiring, coaching, training and evaluating.

  • Demonstrate the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.
  • Applicant must successfully pass required background clearances prior to an offer of employment.

Preferred Qualifications

  • Fluency in second language, preferably Spanish.
  • Knowledge of resources in South King County area.
