CCSCC is Hiring an EPHS Intake Specialist Near San Jose, CA

OVERVIEW:

The focus of the Emergency Programs & Housing Services Division is designed to assist individuals with disaster preparedness, response, recovery, emergency and housing services. Services may include a comprehensive spectrum of support to address barriers to economic self-sufficiency that disaster survivors often experience, housing, rental/deposit assistance, and emergency services.

The Emergency Programs & Housing Services Division serves families experiencing both natural and human made disasters as well as housing services. Programs under the Division are operated by the Program Managers and overseen by the Division Director of the Emergency Programs & Housing Services Division who reports directly to the Director of CAFS.

POSITION SUMMARY: 

This full-time position will provide Intake services to clients seeking housing and/or disaster case management services. The position will determine an individual’s eligibility for the program, assess household needs, input client data into software management systems, and conduct outreach events to promote Emergency Programs & Housing Services Division Programs. Work will involve traveling to multiple areas within Santa Clara County, including San Jose and Gilroy.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Conduct initial screening and assessment using knowledge of triage and tier level assignments for individuals and families identified through outreach, walk-ins, and referrals
  • Maintain contact with clients seeking housing and/or disaster related services through phone, email, video, conferencing, etc
  • Coordinate outreach in conjunction with local public and private community agencies
  • Maintain client intake documentation and forms
  • Establish and maintain positive relationships with individuals and families seeking services
  • Create and maintain the interest list of clients seeking services; ensuring that families with high levels of unmet needs and vulnerable populations receive priority for services
  • Provide support to staff to ensure families/individuals successfully transition from interest list to caseload
  • Ensure all client data is current in software management system(s)
  • Maintain absolute confidentiality on all client information
  • Participate in weekly team meetings.
  • Engage in team building activities.
  • Complete other duties as assigned.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE

  • Minimum of a BA/BS from an accredited educational institution with major course work in social services, public administration, psychology, health sciences, or a related field (equivalent education and experience that demonstrates the ability to perform the job may be considered)
  • Experience monitoring case files in compliance with audit and funder requirements preferred
  • Experience working with vulnerable populations such as individuals with low and fixed income, underserved Santa Clara County residents, including but not limited to seniors, students, and transition-age youth preferred
  • Experience working in a non-profit and experience in community outreach, community organizing and leadership development preferred

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

  • Outstanding oral and written communications skills, including English fluency
  • Bilingual/ bi-literate in English/Spanish required as most clients do not speak English as their first language
  • Must possess a strong work ethic, enjoy working collaboratively, and be capable of working independently with a high level of reliability

  • Excellent basic computer skills (e.g. keyboarding, Microsoft Word Office Suite, Google Suite, Zoom) as well as comfort with using cloud computing applications, electronic health records, as well as learning new technology tools quickly
  • Exceptional organizational, problem-solving and time management skills
  • Excellent multicultural experience and ability to work well with a diverse population
  • Flexible, proactive, adaptable and able to work in a fast-paced, changing environment 
  • Highly developed interpersonal skills, enabling the candidate to work collaboratively with diverse audiences
  • Ability to research, analyze information and represent data in meaningful ways
  • Ability to encourage staff to work collaboratively in high stress environments such as outreach event planning
  • Completion of annual CCSCC certification training to drive agency vehicle

PHYSICAL REQUIREMENTS

  • Ability to sit for long periods of time
  • Ability to lift 25 pounds
  • Ability to drive between agency sites; local travel within Santa Clara County

OTHER QUALIFICATIONS:

  • Criminal background check via livescan fingerprint
  • Must have TB test performed and submit results
  • Provide proof of COVID-19 vaccination and booster status
  • Automobile, valid driver’s license and auto insurance per agency policy; or have access to reliable transportation

WORKING CONDITIONS: 

  • Work is performed at multiple sites within Santa Clara County, including but not limited to, San Jose and Gilroy.
  • Generally in an office environment. Professional appearance and attire expected and required.
  • Must be flexible, adaptable, detailed and resilient as Intake Specialists will be working with those who have suffered tragedies.

HOURS AND OTHER CONDITIONS: 

  • Standard office hours are Monday through Friday, from 8:00 a.m. to 5:00 p.m.
  • This is a full-time, non-exempt position. Some evenings and weekends will be required to meet the needs of the position and program outreach events.
