Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

We are actively seeking a Registered Nurse to join our Tucson team at our Toole facility.

 Job Summary:

The RN works as part of a multidisciplinary team and is responsible for the nursing care provided to patients while on his or her unit (behavioral and medical). The RN is also responsible to carry out orders from the medical practitioner and communicate any barriers to providing service immediately to his or her supervisor and attending medical practitioner.

Nursing Duties

  • Completes admissions/discharges and documents nursing assessments in EMR within one hour of admission/discharge
  • Completes a nurse shift note for all existing patients at least once per shift.
  • Assigns monitoring tool/type that will be used as well as frequency to monitor patient condition.
  • Plans and provides nursing care for patients assigned to him or her on the unit, to include assigning monitoring tools based on patient acuity.
  • Consults with medical practitioners and other assigned staff in designing appropriate care plan for patient.
  • Reviews discharge plan with patient or designee to ensure understanding of all discharge instructions as well as any prescriptions being received at the time of discharge.
  • Administers medication and documents accordingly.
  • Arranges further medical treatment when indicated.
  • Assists in maintaining a safe and secure environment for staff and patients by communicating needs to designated personnel.
  • Maintains accurate records and documentation of patient monitoring, treatment, interventions, laboratory results and other pertinent information concerning the care of the patient while on the unit.
  • Draw blood for labs and collect/process urine samples.
  • Completes medication pass and completes documentation concurrently in the EMR: NextGen.
  • Promotes safe patient environment through on the floor interaction with patients; applies ‘de-escalation’ techniques to help people manage their emotions and behavior
  • Provides patients with education regarding wellness, disease prevention and health promotion.
  • Will cross train in other departments to provide coverage when needed/able.
  • Use professional judgement to ascertain and to apply appropriate intervention techniques with a patient who is in crisis.
  • Communicates effectively with a variety of disciplines, patients, family members and stakeholders.

Functions calmly in situations, which require a high degree of sensitivity and tact Provides prompt, efficient, and responsive service. 

  • Exercises appropriate judgement in answering questions and releasing information; analyze and project consequences of decisions and/or recommendations.
  • Maintains up to date with skill set through continuing education. 

Skills/Requirements:

  • Registered Nurse (RN) Licensed in the State of Arizona; unencumbered. 
  • Bachelors of Science in Nursing, preferred.
  • Behavioral Health, Substance Abuse and/or Psych experience is a plus!
