We are looking for a Peer Support Specialist for our Casa Grande Inpatient Program.
Community Bridges is one of the largest providers of substance abuse/behavioral health treatment in Arizona. We are looking for a Peer Support Specialist for our Tucson Inpatient program. The Peer Support Specialist will assist in the transition to the most appropriate level of care available in conjunction with the established treatment team. This includes ensuring the coordination of care, ensuring that family and other social supports identified in the treatment plan are involved in the discharge planning process. The Peer Support Specialist works in conjunction as part of a multi-disciplinary team made up of a Clinical Supervisor, Emergency Medical Technician (EMT), and an on-call Registered Nurse and/or a Licensed Behavioral Health Professional. In this role, the Peer Support Specialist is the first point of contact with emergency, first responder personnel, and the general public. They are responsible for the coordination of care, transition and continuing care planning, individual and group support, discharge planning, and outreach and follow-up.
