Community Bridges, Inc. (CBI) is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.

We are currently recruiting for a Peer Based Service Coordinator (PBSC) at our Winslow facility.

This position is responsible for ensuring the coordination of care with all those parties who are currently involved in the patient’s case, ensuring that family and other social supports identified in the patient’s treatment plan are involved in the treatment planning process (when involvement is appropriate), as well as addressing some of the DSMIV; Avis IV stressors such as housing problems, problems with accessing health care, problems related to social support, Crisis Intervention, basic life skills, forensic based support, pre-employment or educational problems, and ongoing aftercare.

The PBSC works in conjunction as part of a treatment team made up of a Clinical Supervisor, Licensed Behavioral Health Professional, Outpatient Outreach PSS, Medical Assistant, and other involved stakeholders in the participants treatment. The PBSC also assists the treatment team in completing many of the reporting responsibilities and requirements that are mandated, both internally and externally. They are responsible for the coordination of care, individual and group support, and aftercare.

Skills & Requirements:

• High School Diploma or General Equivalency Diploma (GED)
• Must have 12 consecutive months of recovery from substance abuse and/or General Mental Health
• Must have aLevel 1 Fingerprint Clearance Card
• Must possess a valid driver’s license with good driving history
• Must have a clean 39 month MVR Record
• Must be at least 21 years of age
