Community Bridges, Inc. (CBI) is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.

The RN participates as a member of the team in delivering quality health care to patients, in support of Community Bridges mission, vision and values. The RN provides a variety of nursing services both in person and by phone utilizing the nursing process and leadership skills to address acute and chronic needs of clients. The primary focus is helping members engage in preventative PCP appointments and coordinating on going medical services with the PCP or other specialists as needed. He/She works under the general direction of the designated supervisor. The RN will operate as a RN Case Manager, carrying a case load of members in the program who have identified medical needs. The RN will develop, implement, and evaluate individualized patient care plans. They act as social workers, advocate patient welfare, and serve as a liaison between patients, their families, and healthcare providers.

Skills & Requirements:

• Valid AZ RN License in good standing and without restrictions
