Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

We currently seek Clinical Coordinator for our SMI program , Heritage, in Mesa.

A Community Bridges Inc. (CBI) Clinical Coordinator provides clinical and administrative supervision, clinical oversight, training, and mentoring for Case Managers and Rehabilitation Specialist on an assigned team. Ensures clinical quality and productivity standards are met and exceeded by direct reports. Ensures assessments and treatment plans are developed and implemented for each patient on their team.

ESSENTIAL JOB DUTIES OR RESPONSIBILITIES

  • Coordinates day to day operations of their assigned team.
  • Serves as a point of contact for the team during business hours for community stakeholders.
  • Ensure weekly team meeting is being facilitated in an effective manner.
  • Assesses competency of clinical staff through observation and testing techniques. 
  • Assists clinical staff in developing required competencies, documents training and coaching moments in employee file.
  • Monitors daily activities and coordinates care with supportive members and providers.
  • Assesses patient needs, including assessing for high-risk indicators and documents history/assessment in a manner that is clinically pertinent to the individual patient.
  • Ensures required face to face interaction with each assigned patient is provided and documented accordingly.
  • Participates in treatment planning sessions and assists treatment team by coordinating planning effort.
  • Addresses barriers and concerns with patients, stakeholders and team members.
  • Acts as liaison for program members and their families to various community agencies, hospital consultants and services.
  • Works closely with Clinical Leadership to ensure monthly deliverables and outcomes are being met.
  • Provides back up to the on-call and team members, when needed.
  • Provides services in the field, at the member’s home or where the member is currently located.
  • Provides training of staff on various issues and topics relevant to the specific member population.
  • Documents interaction with each member on a daily basis prior to the end of his or her scheduled shift.
  • Participates in daily staffing and provides insight and updates on the member’s progress as well as recommendations for additional services that will help the member be successful.
  • Provides outreach and engagement activities daily in addition to after hour response while on-call or when member requests assistance.
  • Works with staff to ensure timely discharge from hospital setting by ensuring discharge planning begins at intake.
  • Continues ongoing education and training with internal and external training partners.
  • Participates in petitioning process as determined necessary 24/7.
  • Other duties as assigned.

Minimum Education & Experience Required:

  • Bachelor’s degree in a field related to behavioral health OR a combination of education, training, and/or experience
  • 1 year experience working with the SMI population with additional experience in multiple settings and levels of care, including both mental health and substance abuse
  • 1 year experience in a behavioral health position providing outreach and engagement activities.
  • Ability to pass criminal background check for jail visitation/clearance
  • Valid Arizona Driver’s License with 39 month clear MVR
  • Behavioral Health Technician (BHT) 
  • CPR/First Aid
  • Crisis Prevention Institute (CPI)
