Care Transition Specialist (EMT)

Job Summary :
The Care Transition Specialist (CTS) is a New York State Certified Emergency Medical Technician, with
experience performing patient care tasks in the pre-hospital environment that are consistent with that level of training and scope of practice.
This includes general patient assessment, basic airway and breathing management, CPR and defibrillation utilizing an AED, basic treatment of shock, emergency childbirth, and the care of medical emergencies.
The CTS is proficient in the basic care of victims of trauma that include basic trauma assessment, bleeding control, helmet removal, extremity immobilization, splinting, and spinal immobilization.
The CTS is able to document patient care as part of an electronic medical record.
As a CTS for Cayuga Health, the goal is for the experienced EMT to use their patient care experience to help facilitate successful transitions for patients from the hospital setting to the home care setting.
This transition includes core, evidenced based elements that are implemented to decrease the need for re-hospitalization.
When ordered by Cayuga Health providers, the CTS will provide home visits to collect information and data, assess a patient's condition and / or environment, and support the patient's provider with delivering virtual care appointments in the home setting.
While the ultimate goal of these home visits is to help stabilize the patient in the home care setting, they may also facilitate communication and transportation to the hospital when that is not possible.
Job Responsibilities :
Must have the ability to communicate effectively verbally; over a telephone & radio
Have the ability to interpret written and verbal instructions and guidance
Examine and evaluate patients to determine necessary care
Provide patients with emergency care by utilizing appropriate techniques & equipment
Must be able to read and interpret English language manuals & written instructions; converse utilizing the English language
Maintain patient confidentiality and perform all care with respect for patient rights
Coordinate transportation for patients in accordance with policies & procedures
Possess the ability to bend, stoop, & kneel
Provide teaching and coaching for patients to better manage their own care
Must be willing to work shifts typically lasting between 10-12 hours
Have the ability to perform duties in all environmental conditions including heat, cold, & precipitation
Use evidence based techniques for teaching
Must be able to document patient care utilizing an electronic medical record (EMR)
Must be own and operate a motor vehicle in accordance with NYS rules & regulations
Must successfully maintain state EMT licensure; CPR certification
Other Responsibilities :
Completion of Care Transitions Training
Understanding of Teach Back technique
Understanding of basic Motivational Interviewing technique
Understanding the impact of health literacy on patients
Understanding the impact of social determinants of health on health careoutcomes
Completion of MedWand training
Physical Requirements :
The position requires a mixture of sitting (20%), standing (30%) and walking (50%). Individual must possess visual / auditory ability sufficient to comprehend written / verbal communication and the ability to clearly communicate to others. As well as :
Ability to perform tasks involving physical activity, which may include heavy lifting (the ability to transfer and position a patient that weighs 200lbs) and extensive bending and standing.
Ability to deal effectively with stress, both physically and mentally, while maintaining a professional attitude and reliably performing required duties.
Ability to work the hours necessary to accomplish job duties.
Qualifications
Current EMT certification / license in the state of practice
Current CPR certification
Must possess a valid driver's license in New York State
Two years of experience working full-time as an EMT on an ambulance (preferred)
Ability to bend, pull, push, and lift up to 50 pounds
Ability to follow oral and written instructions
Demonstrated ability to implement clinical care in accordance with policies & procedures
Ability to exercise sound judgment while working in a stressful environment
