Cayuga Medical Center is Hiring a Case Manager Near Trumansburg, NY

Job Summary:
Provides a range of clinical, administrative, and support services to assure effective care
and programming. This position requires a high degree of flexibility and a willingness to
multi-task from day to day. Works at the direction of the Clinical Supervisor to support
the Primary Addiction Counselor in delivering clinical services.

Minimum Qualifications:
One year of relevant experience, an Associate's degree in SUD or related field or a
combination of experience and education. A valid NYS Driver's license with an
acceptable driving record for the past 5 - 7 years.

Desired Qualifications:
Bachelor's degree in SUD or related field, CASAC-T, 3 years relevant experience,
experienced in residential chemical dependency treatment in addition to minimum
qualifications. Knowledge of motivational interviewing desired.
Skills & Abilities: A basic understanding of addiction and recovery issues as well as
knowledge of basic counseling techniques and group facilitation are essential.
Knowledge of strength-based principles of treatment, discharge planning skills, and use
of client centered plans of care. Excellent interpersonal skills, crisis intervention skills,
ability to maintain healthy and appropriate treatment boundaries, basic use of a computer
for documentation and communication, ability to collaborate proactively with team
members. Ability to perform individual and group treatment modalities, multi-task,
maintain healthy and appropriate treatment boundaries, and ability to be flexible.

Key Tasks:
• Supports the Primary Addiction Counselors (PAC) in meeting the demands of a
client case load by carrying out interventions outlined in clients plan of care, thus
allowing the PAC to focus on clinical aspect of the client's services.
• Supports a therapeutic milieu as a presence in the community, enforces rules of
the program with empathy, reinforces positive client behavior, and assist clients in
adapting to the program structure.
• Completes all routine paperwork and documentation within established time
frames in accordance with agency practices

Job Duties:
1. Meet with client upon initial intake, or within 24 hrs, to review the admission
agreement and complete Emergency Discharge Plan, and the PAS44.
2. Provide client orientation to program rules and expectations.
3. Schedule separate 1:1 appointment as needed or identified in plan of care and
explore discharge planning options available with client.
4. Make referrals to identified community-based services and assist client in
completing consents to release information to housing options and other postdischarge care providers.
5. Supervise client in scheduling and completing phone interviews/screenings as
needed for discharge planning
6. Meet with client on an as needed basis to complete interventions outlined in plan
of care.
7. Ensure arrangements for continued care is scheduled to begin within 3-10 days
following patient discharge. This will include, but not limited to Outpatient
appointments, MAT options, self-help meetings, transportation information,
geographical maps for clients, halfway house interviews, and transfers to other
facilities.
8. Arrange transportation for clients, as needed.
9. Facilitate applications of eligibility for health insurance and other available forms
of public assistance, as well as resumption of benefits for clients awaiting release
from the facility.
10. Teach social skills in the milieu, modeling pro-social adult decision making,
remain the calmest person in the room while interacting with clients, and when
appropriate look for teachable moments to help clients learn from their behavior
within the milieu.
11. Supervise client activities on the unit and maintain presence in community and
common areas
12. Facilitate groups using established curriculum.
13. Attend morning huddle and all scheduled treatment planning meetings and
actively participate as part of the treatment team.
14. Complete the discharge forms as required by CARS to presented to the primary
counselor for their discharge session with client.
15. Safely transport clients to and from medical appointments, court appointments, or
volunteer events as needed. Supervise clients during activities off site.
16. Identify success stories to share - may include inviting past clients to come to
facility and share their story of success with supervisor approval.
17. Attend all required in-services, trainings, and clinical department meetings.

19. Maintain compliance with all Cayuga Addiction Recovery Services policy and
procedures.
20. Maintain confidentiality, as per agency practices and confidentiality law.
21. Maintain compliance with all OASAS regulations and standards.

Other Tasks & Responsibilities:
• Monitor and maintain the safety and security of the clients and the facility.
• Maintain a safe community; keep an environment free of substances, minimize
risk of contraband entering the building, be accountable for knowing client and
visitor whereabouts, and follow emergency protocols.
• Assist with admission and discharge procedures.
• Attend staff and team meetings as assigned
• Supervise client mealtimes and food preparation as needed.
• Planning and conducting special activities, as assigned
• Other duties as assigned
