A PPS Coordinator is responsible for the timely and accurate completion and submittal of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) for each rehabilitation inpatient. The PPS Coordinator is responsible for continual data collection and management of information for CMS and NYS Department of Health. Accuracy of Quality Indicator (QI) scores is reviewed by the Department Director. The PPS Coordinator assists in the ongoing monitoring of identified benchmarks for the inpatient population. The PPS Coordinator has access to confidential patient/ employee/ organizational information and maintains confidentiality of all information.

Position Overview:

  • PPS Data Entry and coordination of referrals. Accepts and logs admission referrals (and status through process) for pre-screening assessment for admission to acute rehab.
  • UDSMR Software Management. Oversees the running of the UDS-PRO software. Including coordinating submission of reports to Director for review. Maintains a current knowledge of the UDS-PRO software and provides expertise on such within the inpatient rehabilitation setting. Ensures that the IRF-PAI reflects the functional status and 24 hour care needs of each patient to result in the correct Case Mix Group (CMG) assignments and proper billing. Enters IRF-PAI data into the UDS-PRO computer software as required by CMS and the fiscal intermediary and attests to the accuracy and completeness of all the data
  • Organization of Department. Ensures organization of department to include faxing, ordering supplies, management of forms, bulletin boards etc.
  • Admission Pre-Assessment Coordination: Coordinates arrivals to acute rehab with admissions and interdisciplinary team.
  • Tracking of Quality Indicator completion with coordination of responsibilities. Coordinates the completion of the admission assessment of the IRF-PAI by the fourth day of the patient's stays and the discharge assessment by the 5th day following discharge for each inpatient rehabilitation patient. May require discussion/interaction (written or verbal) with various staff members concerning any differences in reported functional status to complete the IRF-PAI.
  • Schedules and ensures electronic transmission of the IRF-PAI according to Prospective Payment System/ Federal guidelines.
  • Completes concurrent audits of IRF-PAI's and clinical documentation to assure accuracy and consistency of documentation.
  • Performance Improvement reports for Acute Rehab. Manages data collection and entry for monthly PI report reviewing variance with department Director prior to submission.
  • Assist with Insurance company updates. Supports communication of information to and from Insurance companies, Home Care agencies, and Equipment Vendors after INITIAL consult or approval is completed by nurse coordinator, therapist or responsible party.
  • Maintains updated information regarding facility compliance with federal regulations regarding what is commonly referred to as the "60% Rule" and disseminates this information regularly to appropriate management staff. (Develops and maintains data spread sheet)
  • Coordinate with Medical Records Coders. Identifies admission and discharge assessment reference dates and appropriate impairment groups (under supervision of Director) for the completion of the IRF-PAI for all adult patients. Understands the Case Mix Group (CMG) Classification System and assures the appropriate documentation to support the treatment of the patient. Completes transfer of CMG to medical records. Coordinates coding with hospital coders.
  • Assists the Director, nursing and other appropriate departments in identifying and monitoring trends in patient care and program efficiency as identified through the IRF-PAI.
  • Assists the Director by providing outcomes reporting as requested.
  • Works closely and collaboratively with all members of the clinical team to ensure compliance at all levels.
  • Actively participates in administrative meetings within service area.
  • Maintains updated and ongoing knowledge of federal regulations concerning the Perspective Payments System and the IRF-PAI to assure compliance with all regulations.
  • Fosters positive internal and external relationships with all customers (staff, co-workers, administration, medical staff, patients, visitors, community).
  • Miscellaneous Duties and Responsibilities. Performs all other duties as requested.

Minimum Qualifications:

  • Intermediate computer skills in Microsoft Word and Excel
  • Ability to handle multiple priorities
  • Strong attention to detail
  • Strong interpersonal skills
  • Strong organizational skills
  • Demonstrated ability to coordinate efforts within an inter-disciplinary team

Education: Associate Degree in Business, Technology, or Administrative Assistant

Experience: 5 years of Experience in Health Care Environment

Licensure: Not applicable.

Certification: BLS

Physical Requirements: Repetitive use of hands and fingers (e.g., use of computer keyboard); may require lifting and carrying light loads (up to 40 lbs), including boxes, equipment, IV solutions and stooping and kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves and to file documents in lower file drawers); sitting, walking, or standing for long periods of time may be necessary; must be able physically to operate the equipment used for the job.
