Catholic Home Care Field RN will assess, plan, develop and implement with the patient or responsible caregiver a comprehensive home health plan of care for nursing and identifies patient needs for other disciplines and services. The RN Case manages, coordinates and evaluates the plan of care with therapeutic, social service, pastoral care, home health aide services, other organizations and physicians for an assigned caseload. The RN instructs patient or responsible caregiver in nursing care, disease management, health promotion as directed by the physician's plan of treatment and consistent with Agency regulatory and professional standards.
Qualifications:
The pay for this position ranges are below:
$35 per hour for Orientation
$30 per hour for In-service
$135 Start of Care initial visit rate
$65 revisit rate
$135 Start of Care rate (Saturday/Sunday)
$65 revisit rate (Saturday/Sunday)
$150 Start of Care rate (Holidays)
$75 revisit rate (Holidays)
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
