Our Care Management department is looking for Social Workers as well as Registered Nurses for Transition of Care to contribute to their team of people who love the work they do. These newly created positions will help with Length of stay initiatives across all of Catholic Health!

If you are a Social Worker or an RN with at least two (2) years of experience within a healthcare setting, possess a Bachelor’s degree, and have good knowledge of services within the immediate community and/or ability to locate those not easily identifiable this is the job for you!!!!
