Catholic Charities Serving Central Washington is Hiring a Home Visitor Lead - ECLIPSE Near Yakima, WA

Position Summary:

The ECLIPSE Home Visitor Lead will provide leadership and guidance to the ECLIPSE Home Visitor team members by providing support and supervisor. The ECLIPSE Home Visitor Lead will have formal supervision and staff training responsibilities, including performance evaluations, new staff onboarding, and other supervisory duties that enable them to effectively support and monitor the work of their direct reports. The ECLIPSE Home Visitor Lead will have interactions with parents and caregivers with whom their Home Visiting Team serve in order to provide quality assurance of program goals.

Responsibilities:

Direct Supervision of Home Visitor Team

  • Provide training and support of new and experiences staff in their Home Visiting Role
  • Collaborate with their supervisor (Program Manager) to ensure quality of service being provided by Home Visiting Team
  • Provide assessment of skills of Home Visiting Team and complete performance evaluations yearly
  • Monitor Home Visitor Documentation and Calendars
  • Provide weekly supervision of direct reports
  • Build and maintain professional collaborative relationships with Lead positions within Early Learning initiatives within Catholic Charities
  • Maintain professional working relationship with child care providers, community partners and program stakeholders

Provide ongoing case management and manage a caseload of ECLIPSE children and families

  • Provide home visits with parents of ECLIPSE children at a minimum monthly to address positive parenting skills and implement behavior plans consistent with the classroom.
  • Implement identified Evidences Based Practices consistent with the treatment plan.
  • Monitor family’s need for ECLIPSE transportation and communicate with transportation liaison and document in child record.
  • Provide classroom support and follow up with classroom staff regarding classroom interventions, children’s progress or concerns, and family needs.
  • Make referrals to community services whenever necessary.
  • Meet productivity standards of 50% of time in direct face-to-face services including home visits, MDT meetings, case management, classroom observations, etc.
  • Maintain a caseload of 5-10 families
  • Participate in family intake assessments completed by the Mental Health Consultant with caregivers and child; observing parent-child interactions
  • Facilitate entry of child into the program collaborating with classroom staff and transportation (as needed)
  • Complete Enrollment paperwork

Facilitate multidisciplinary meetings with parents, sources of referrals and allied service providers to establish a treatment plan for the child.

  • Facilitate MDT meetings every 90-days to review the treatment plan and progress or concerns in services.
  • Ensure informal supports, schools, and allied services providers are included in the MDT.

Complete all documentation according to agency policy and required by the contract.

  • Document all home visits, community meetings, MDT meetings and other contracts on calendar daily
  • Complete documentation within 48 hours of scheduled service
  • Keep ELMS/Documentation tracker updated as required
  • Attend and plan for transition meetings in conjunction with Mental Health Consultant upon family’s exit from ECLIPSE
  • Document all phone calls, case notes, contacts with allied service providers and other treatment information in the Electronic record according to the ECLIPSE policies and procedures.
  • Maintain communication with assigned social workers and referral sources regarding progress and concerns
  • Follow up with classroom staff to make sure objective assessments are completed and scored on time.

Attend and participate in staff meetings, trainings, and individual and group supervision.

  • Communicate directly with supervisor with any questions regarding the formulation or implementation of behavior management plans, treatment, etc
  • Participate in monthly supervision meetings with ECLIPSE Program Manager
  • Support facilitation of ECLIPSE Team Meetings in collaboration with ECLIPSE Program Manager
  • Staff cases that require team coordination or therapeutic input.

Observe confidentiality in all related program functions with staff and families.

Organize and participate in family events and parent meetings.

Perform other duties as assigned


Job Requirements:

The following requirements are those that are normally required for performance of this position. Physical Requirements:

This position normally requires the physical demands of standing, walking, bending, lifting or performing other work requiring medium physical exertion, talking and hearing and adequate vision on a regular basis to perform the job requirements. These physical demands are required up to 60% of the time. In addition, approximately 40% of the time may include lifting and carrying children, moving quickly around the room to maintain appropriate supervision and safety and sitting on the floor or in small chairs with the group.

Non-physical Requirements:

Education

  • Bachelor’s degree in human service field from an accredited University required

Experience

  • 2 years’ experience working with caregivers and at-risk children

Special Skills

  • Posses leadership skills
  • Possess strong behavioral management skills
  • Possess good verbal and written communication skills
  • Knowledge of child abuse and neglect issues, home visiting, parenting skills training, mental health and community resources.
  • Ability to work independently and as a team member
  • Ability to represent the agency in a professional manner within the community
  • Ability to work in a collaborative manner with referral sources
  • Complete accurate paperwork in a timely manner
  • Basic computer proficiency

Licensure, Registration, Certification

  • Agency Affiliated Counselor certification
  • Complete Child Abuse and Neglect training
  • Valid Washington State driver’s license and minimum required liability insurance
  • Must be deemed insurable as determined by Catholic Charities liability insurance provider

Employment Conditional Upon:

  • Being cleared by criminal background check and fingerprinting when required
