Bilingual in English/Spanish Required - Additional $1.00 per Hour (Must Pass Agency Bilingual Test)

The Outreach Worker II/Case Manager II (Bilingual) is responsible for performing outreach to people experiencing homelessness county-wide. The Outreach Worker II/Case Manager II (Bilingual) reaches out to individuals and families that are not being served or are underserved by existing community service delivery systems, practicing trauma informed care, building trusting relationships, and developing a rapport to lead to openness in seeking help and acceptance of assistance.

The Outreach Worker II/Case Manager II (Bilingual) engages individuals experiencing homelessness as a first step to accepting care for immediate health and safety needs, using available services and resources, and taking steps to become integrated into the community.

The Outreach Worker II/Case Manager II (Bilingual) seeks to enable individuals and families to accept placement and opportunities to stabilize their lives by utilizing diversion practices, crisis response and intervention, shelter referrals, interim housing placement, and linking individuals to appropriate continued services.

The Outreach Worker II/Case Manager II (Bilingual) continues to work with the participants who engage in shelter services, providing resources and support to stabilize the participant, assess the participant’s needs, and refer the participant to appropriate support services. The Outreach Worker II/Case Manager II (Bilingual) will focus on keeping the participant in shelter with the goal of document readiness and permanent housing. When appropriate, the Outreach Worker II/Case Manager II (Bilingual) will transition the participant to the assigned case manager for their shelter stay and continue to support the assigned case manager. The Outreach Worker II/Case Manager II (Bilingual) will engage a participant for HOST Rapid Rehousing in a housing search, interacting with landlords, and completing Rapid Re-housing paperwork and check requests as appropriate. The Outreach Worker II/Case Manager II (Bilingual) often works outside of the office to meet program participants at an assigned program site.

Essential Functions

  1. Conduct mobile outreach and create client service engagement opportunities for homeless individuals and families on the streets.
  2. Follow Engagement, Assessment, and Referrals (EAR) procedure as defined in Catholic Charities’ Outreach Services (OS) Policy and Procedures.
  3. Facilitate and maintain community relationships to create referral pathways for program participants into Catholic Charities’ programs. Maintains a network of external partners and professional relationships and communicates effectively with all internal and external partners using shared tools provided by the agency.
  4. Assist with screening of potential program participants for the Homelessness Prevention and Rapid Re-Housing programs.
  5. Provide program materials and referral training sessions to community partners regarding shelter services, including outreach to motels, churches, and downtown merchants.
  6. Provide support and guidance to participants as they transition from street to shelter.
  7. Follow safety protocols for community street outreach.
  8. Perform job functions in line with the policies and procedures guiding the work of this position, the program, and the agency.
  9. Research and implement best practices in street outreach and assist participants in acquiring resources for acquiring housing.
  10. Develop and manage a comprehensive outreach/working relationship with all stakeholders, elected officials and staff, commercial and private community activists, and clients.
  11. Divert or link individuals and families with the appropriate services, including Coordinated Entry, diversion, crisis response and intervention, shelter referrals, temporary housing, permanent housing, and Rapid Re-Housing.
  12. Provide thorough and complete documentation of all required activities and engagements, per contract requirements, including Problem Intervention Response Plan (PIRP) case notes, and documentation of reasonable accommodation requests.
  13. Assist with housing location, Rapid Re-Housing documentation and place check requests when appropriate.
  14. Serve on an agency or community service committee representing the program or the agency.
  15. Conduct outreach worker related duties independently and exercise sound professional and service intervention judgement, requiring minimal documentation errors corrective actions.
  16. Conduct screening and assessment with participants with the objective of determining housing barriers, and appropriate housing intervention.
  17. Practice assertive engagement with individuals and families through persistent and creative activities to motivate, encourage participation, and promote follow-through.
  18. Provide team support to supervisor, shelter staff, and participant advocates to support participant’s success.
  19. Maintain up-to-date and thorough Case Records and document service encounters as required, input data in a timely manner in Homeless Management Information System (HMIS), DataLink, and/or Smartsheets.
  20. Respond to calls, e-mails, update requests, and other inquiries in a professional and timely manner.
  21. Conduct case management related duties independently, exercise sound professional and case management judgement, maintain Case Records with minimal documentation errors and/or corrective actions.
  22. Support the Mission, Vision, and Values of Catholic Charities in the performance of all essential job functions and responsibilities.

Note: Catholic Charities considers this position to be a mandated reporter of child abuse and elder abuse.

Other Responsibilities

  1. Work with Diocese of Santa Rosa and local parishes as appropriate and requested.
  2. Perform other related duties as assigned.

Education, Experience, and Skills Required

  1. Associate’s degree in a social service or related field, one to two years’ experience as a case manager in housing stabilization, and one or two years of experience working as an Outreach Worker I, or equal work experience required. An additional three to four years combination experience as a case manager and in street outreach or equal work experience may be substituted for an associate’s degree. Bachelor’s degree preferred.
  2. Bilingual (English/Spanish), passing score on the Catholic Charities’ test of oral and written Spanish fluency required.
  3. Experience in crisis intervention and problem solving with ability to diffuse an intense situation.
  4. Understanding of causes of homelessness, experience working within social services, street outreach methods, and familiarity with relevant community resources. Knowledge of Housing First principles and methods preferred.
  5. Completion of training and/or certification to be able to provide specialized services and increased performance as an outreach worker.
  6. Must become certified in CPR and First Aid within six months of transfer to this position or hire date.
  7. Ability to work under pressure with a positive, solution-oriented approach. Experience in crisis intervention and problem solving with ability to diffuse an intense situation.
  8. Ability to learn and apply various engagement techniques to a variety of situations.
  9. Cooperative, friendly, and helpful attitude with clients and co-workers. Ability to work closely with other employees to ensure positive, constructive environment within the program and throughout the agency.
  10. Computer literacy required, including experience with Office 365 and Excel.
  11. Strong written and verbal communication; organizational, conflict resolution and problem-solving skills.
  12. Passion and enthusiasm for the mission of Catholic Charities and its clients.
  13. Valid driver license and at least state required minimum of automobile insurance, clear DMV driving record and ability to provide own transportation. Must be willing and able to travel between agency sites.
  14. Must pass DOJ background clearance (fingerprinting) in accordance with the Diocese of Santa Rosa’s policies, prior to start of employment.
