1. Provides case management services to women and infants in high risk communities, handling complex situations and conducting home visits. May work evenings and weekends.
2. Completes all participants information and updates information in the Cornerstone database system within the prescribed time frames. Provides accurate and timely recording of all home visits and all other required data according to the required guidelines, policy and procedures of the agency and the funder.
3. Maintains an educational and supportive relationship with clients.
4. Maintains up-to-date and accurate client files and in documenting clients information in the cornerstone database, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing, or accrediting bodies in the course of performing job expectations.
5. Engages clients in services and facilities referrals to appropriate services when needed.
6. Handles all crisis situations and client and complaints with sensitivity and experience, makes independent decisions and consults with supervisory chain.
7. As a mandated reporter, immediately notifies the supervisor/director and the Department of Children and Family Services (DCFS) of any suspicions and findings of abuse or neglect.
8. Identifies new resources and makes suggestions regarding utilization of current resources to the advantage of participants. Collaborates with service agencies on behalf of the participant. Maybe assigned to conduct tours of the program, responds to inquiries from other agencies, visits agencies that are potential referral sources, and attends community meetings.
9. Conducts community outreach in order to identify women and children needing services and provides training and workshops related to the services provided.
10. Participates in continuous quality improvement of services.
