The Homeless Case Worker Position will focus on providing support to ensure homeless school-aged children, residing in hotels, are receiving adequate education. They will assist with enrollment, transportation and other coordination as appropriate.
The Homeless Case Worker will also perform all aspects of case management with families staying in the hotel and families referred from DSS/211 for services, including but not limited to: Transportation of clients between the hotel and shelter, transport clients moving from the Emergency Family Shelter to housing destination. Transport clients to appointments as needed and help transport clients to apartment viewings. Engage in client interviews to assess and understand the nature and extent of their challenges, which may include personal and family adjustments, potential foster care situations, financial struggles, employment issues, basic needs concerns, and physical or mental impairments. They will gather relevant information about the client's medical, psychological, and social factors, evaluate their capacities, and provide counseling and support to develop plans for meeting their needs and improving their social functioning. The Homeless Case Worker will actively refer clients to community resources and organizations, maintain accurate documentation and reporting for government agencies and funding sources, and ensure effective communication with supervisors and other stakeholders. Timely reporting of safety issues is also an essential responsibility.
This position pays $21.00 Per hour
