St. Margaret’s Center in Lennox is a program of Catholic Charities of Los Angeles that serves as a connection point to L.

A. County’s Coordinated Entry System (CES) program in the South Bay. The Client Resources Coordinator II position will provide homeless services street outreach, connection to the CES system, and housing navigation case management for people experiencing homelessness in the city of Hawthorne.

Responsibilities include, but are not limited to :

• Provide street outreach services to homeless households in the city of Hawthorne.
• Assess clients’ situations and needs (including utilizing CES assessment tools), and provide case management in regard to achieving their personal goals.
• Provide system navigation services with clients, assisting them in securing needed documents and linking them to appropriate benefits and support.
• Meet regularly with other case managers / navigators on the SPA 8 team to coordinate services.
• Assist with advocating for housing opportunities for as many homeless households as possible. Depending on need, housing may be interim housing, permanent housing, or other types of shared or assisted housing programs.
• Support City of Hawthorne Homeless Services Supervisor with additional job duties as required.

Qualifications :

• Bachelor’s degree preferred, plus one to three years of related experience
• Six months of case management.
• Bilingual (Spanish / English) desirable.
