Catholic Charities Maine Behavioral Health Network in Portland, Maine has an opening for a Full-Time Adult Opioid Health Home Care Coordinator.
Definition: The Adult Opioid Health Home (OHH) Clinical Care Coordinator shall be a trained, supervised professional who will work as an integral part of the OHH Team to coordinate and monitor the delivery of effective, integrated behavioral and physical health services to enrolled OHH members. Shall provide clinical support to include individual and group sessions focused in the areas of treatment planning, relapse prevention, safe approaches to harm reduction, and motivation along with coordination and management of the overall integration of the member’s emotional and physical health supports and services toward assisting them in achieving their optimal health/wellness goals.

Schedule: Days, Monday – Friday
Starting Salary: $23.50/hour
