Catholic Charities Indianapolis Refugee Resettlement is Hiring a Preferred Communities Case Manager Near Indianapolis, IN

Overview:

We are seeking a dedicated Case Manager to join our team. The Preferred Communities Case Manager provides intensive case management services for individual and family cases that have been identified at pre-arrival as having a serious mental and/or physical disability or have experienced significant adjustment barriers after arrival and are referred to PC program. The PC Case Manager works with clients so that they may overcome barriers to self-sufficiency and quality of life.

Responsibilities:

-Provides comprehensive intake and develops self sufficiency plan, unique to each clients needs.

-Works closely with individuals and families to provide access to services for psychosocial, mental health and medical support, providing assistance as needed for client needs.

-Works with client and service providers to provide access to appropriate mode of therapy (art, music, traditional talk therapy), given each clients needs.

-Uses needs assessment to develop objectives to help each client address said barriers. Works with client to develop timeline in which those objectives are accomplished.

-Performs regular home visits to monitor client progress, boost life skills and work towards self-sufficiency

-Promotes interim solutions for self-sufficiency by directing client to community resources such as rent assistance, reduced-cost services, classes, health seminars and others, as needed

Skills:

-Ability to communicate with team members to assess client needs.

-Ability to travel regularly to corporate office in Indianapolis, Indiana and surrounding community is required.

-Self-motivated. Ability to organize and manage workload to meet deadlines.

-Ability to handle multiple projects and priorities simultaneously.

-Ability to establish and develop positive connections with people of diverse backgrounds and languages, showing respect for people and their culture.

-Understanding of the strength of clients from vulnerable backgrounds as well as awareness of the role systems play in the advantages and marginalization.

Education, Training or Experience:

-Bachelor’s Degree in Social Work, or similar field preferred or related work experience, a combination of education, training and experience is equivalent.

-Experience working with diverse populations is preferred.

-Awareness of Community Resources.

-Background screening required.

-Valid driver’s license and personal transportation vehicle is required.

Pay: $19.00 - $20.50 per hour
