Catholic Charities Diocese of Paterson is Hiring a HOPWA Case Manager (Monday, Wednesday, Friday 9:00am-5:00pm) Near Paterson, NJ

We provide innovative and compassionate services to individuals struggling with substance use and co-occurring disorders. We are looking for dedicated, caring and compassionate people to work in our Residential/Outpatient Treatment Facilities and Community Programs.

HOPWA Case Manager

Schedule per week: Monday, Wednesday, and Friday 9:00am-5:00pm

Location: Paterson, NJ

Job Summary:

Provides Case Management for persons with HIV/ AIDS and their families under the Housing Opportunities for Persons with AIDS (HOPWA) Program. The HOPWA program is meant to prevent homelessness and to improve the quality of life those infected and affected by AIDS.

Responsibilities:

  • Interviews prospective clients in order to determine the eligibility of the client for services and what services are required.
  • Provides eligible clients with vouchers for the following services: Apartment security deposits, back rent (must have eviction notice), utility bills (must have shut off notice) food, household goods, furniture and appliances.
  • Facilitates client access to other community resources available to those with HIV/ AIDS.
  • Documents all client encounters and contacts made on the behalf of clients; completes and submits billing documentation as appropriate; and maintains comprehensive client files.
  • Maintains and reports applicable statistics regarding programs and client services.
  • Performs miscellaneous job -related duties.

Qualifications:

High School diploma. Proficiency in English Language; Must be computer literate. Ability to gather data, compile information, and prepare reports. Knowlege of community resources for person with AIDS. Strong interpersonal/communication skills and ability to work effectively with target population.
