Catalyst Health Group is Hiring a PRN Advanced Practice Provider Near Plano, TX

Lightpath Health is a 501(c)(3) nonprofit organization dedicated to developing and investing in programs that expand access to primary care for those without health insurance in the communities that need it most.

JOB SUMMARY

The PRN Advanced Practice Provider is responsible for conducting wellness visits and evaluating and treating patient's illnesses and providing education and counseling to support them in understanding their overall health or mitigate symptoms and advancement of diseases. Under physician supervision, the Advanced Practice Provider will promote a team-based approach, leading integrated health services teams, to advance program initiatives to improve health and clinical outcomes for the managed populations. Working closely with leadership, the Advanced Practice Provider will support the implementation and advancement of these programs. They will foster strong, positive relationships internally and externally to achieve program goals, including working within cross-departmental teams to execute program goals through patient outreach and providing of health services.

Role and Responsibilities

  • Performs in-person office or home visits, and virtual visits based on patient need and program requirements
  • Conducts health assessments, including recording complete medical and psychosocial history
  • Physically examines patients, records symptoms and makes diagnoses
  • Performs annual wellness exams, including well woman exams and pap smears
  • Develops treatment plans that may include medication and other therapies
  • Provides appropriate treatment and care under the scope of the Nurse Practitioner or Physician Assistant license
  • Provides patient education to promote habits that will prevent diseases and maintain good health
  • Utilizes program tools and resources to support an integrated approach to the managed populations' health and wellness and chronic condition management
  • Communicates and collaborates with other healthcare providers, support staff, and volunteers, including but not limited to physicians and nurses
  • Orders and interprets laboratory test results
  • Follows up with patients to determine the effectiveness of recommended treatments
  • Identifies patients appropriate for Care Management by use of targeted chronic conditions, level of care, and recognition of member's disease specific and preventative measures, knowledge base or deficits in monitoring health, wellness, and chronic conditions
  • Supports quality and utilization initiatives through patient outreach and education, furthering program goals and positive patient outcomes
  • Supports after hours call needs in conjunction with Call Center clinical support staff triaging patient needs
  • Collaborates with Chief Medical Officer, Medical Director or Supervising Physician when appropriate to ensure appropriate, coordinated service delivery
  • Collaborates closely with leadership and management for community health program execution
  • Builds relationships with community health affiliated care sites and partners
  • Provides education and communication of the program offerings
  • Offers solutions to problems and brings to appropriate leadership for resolution
  • Works across functions with peers and external entities to ensure collaboration for shared program goals
  • Professionally always represents the organization; supporting the mission, goals, and objectives; constantly edifying other team members; setting an example of high personal and professional conduct for employees
  • Identifies and reports quality of care issues to the appropriate management and Medical Director
  • Support in creation and documentation of program workflows, policies, and procedures
  • Strategic planning with leaders to bring innovative processes, reporting and communication integration that support positive patient outcomes and overarching program goals
  • Supports off site community health and educational events through planning, set up, tear down, and staffing of event as needed

Qualifications

  • Must possess a current and active Nurse Practitioner or Physician Assistant license without restrictions to practice in the state of Texas.
  • BLS certification for Health Care Professionals required
  • Must maintain valid driver's license, vehicle, and insurance requirements
  • Minimum of 5 years' experience in family medicine or primary care setting
  • Ability to work with a diverse patient/family population
  • Fluency in English and Spanish highly preferred
  • Community health and non-profit interest or experience preferred
  • Value based care and team-based approach interest or experience
  • Experience providing virtual care, including performing telehealth visits, preferred
  • Possesses current knowledge of disease pathophysiology, psychosocial issues, and treatment
  • Chronic Care Management (CCM) experience highly desirable
  • Ability to handle multiple demands of diverse workload and prioritizes critical issues
  • Excellent communication and interpersonal Good oral, written and presentation skills.
  • Ability to build and foster effective collegial relationships, including with clinical staff, co-workers, volunteers, vendors, and community partners.
  • Computer literate with experience in Microsoft Office Suite and Electronic Health Record documentation
  • Able to develop, implement, communicate, and evaluate a plan of care for each patient
  • Strong organizational skills and ability to work both independently and as part of a team

physical Requirements

  • Must have reliable transportation and willing to travel within DFW area to the care site
  • Must be able to stand for various lengths of time
  • Must have stamina to perform manual duties that require strength, such as bending, standing, reaching, and lifting up to 30 lbs.
  • Must be able to tolerate heat/outdoor working atmosphere
