Job Summary:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive and coordinated care to patients, ensuring their needs are met throughout their healthcare journey. Your expertise in acute care and case management will be crucial in delivering high-quality patient care.
Responsibilities:
- Conduct comprehensive assessments of patients' healthcare needs and develop individualized care plans
- Collaborate with interdisciplinary healthcare team members to coordinate and manage patient care
- Monitor patients' progress and make necessary adjustments to the care plan as needed
- Provide education and support to patients and their families regarding their medical condition and treatment options
- Utilize medical records and diagnostic evaluations to inform decision-making and ensure accurate documentation
