Description

This is a floating position, working at multiple residential facilities. The schedule is Tuesday, Wednesday and Friday, daytime business hours.

About the Role:
As a Counselor III, you'll play a pivotal role in providing case management and clinical services to our residents, empowering them on their journey towards recovery and transition to a less restrictive setting. Collaborating closely with an interdisciplinary team, you'll implement tailored treatment plans and interventions, ensuring a client-centered approach that celebrates individual strengths and fosters growth.

Key Responsibilities:

  • Provide therapeutic interventions and counseling, equipping residents with essential skills for daily functioning and symptom management.
  • Collaborate with the treatment team to implement individualized treatment plans and monitor progress towards goals.
  • Conduct mental health assessments and generate treatment plans, ensuring continuity of care and support for residents.
  • Coordinate with external stakeholders, including medical teams and community agencies, to facilitate seamless continuity of care.
  • Stay abreast of best practices and evidence-based approaches in behavioral health, contributing to a culture of continuous improvement and excellence.

Qualifications:

  • Master's Degree in a behavioral science field.
  • Eligible to become registered or certified as a Qualified Mental Health Professional (QMHP) through MHACBO or another state licensing board.
  • Previous experience in case management, preferably with SPMI populations.
  • Strong understanding of Trauma-Informed Care principles and evidence-based practices.
  • Excellent communication and interpersonal skills, with a commitment to cultural sensitivity and inclusivity.
