Job Overview: As a Residential Case Manager, you'll play a crucial role in assisting clients access vital services and supports. You'll serve as an advocate, working closely with clients to understand their needs and goals, and help them navigate the path to recovery. Join us in promoting integrated care, trauma-informed approaches, and creating an inclusive, safe environment for all.
Essential Duties:
* Provide mental health case management and skills training to clients as indicated in their treatment plans.
* Collaborate with the treatment team to assess needs, support development, and facilitate transition planning.
* Deliver individual and group services, including skills training, activity therapy, and crisis intervention.
* Integrate best practices such as Person-Centered Approach, Trauma Informed Care, and Motivational Interviewing.
* Establish and maintain cooperative relationships with outside providers to secure benefits and resources.
* Coordinate treatment and continuity of care with community stakeholders to maintain communication and support.
* Ensure compliance with regulations, policies, and procedures, maintaining accurate documentation.
* Provide guidance, support, and training to other staff to align with our company mission and values.
Qualifications:
* QMHA: Bachelor's degree in a behavioral science field from an accredited college or university or three years of full-time professional experience providing direct service to individuals in a social service setting. Demonstrate the competencies to become credentialed as a Qualified Mental Health Associate (QMHA), as outlined in the Oregon Administrative Rulings (OARs). Certification must be kept current and in compliance with OARs through MHACBO.
* Minimum of 1 year experience working with adults with severe and persistent mental illness.
* Strong communication, interpersonal, and organizational skills.
* Proficiency in Microsoft Office suite and computer operations.
* Valid driver's license and ability to pass DMV check and defensive driving course.
* Ability to perform Basic Life Support and use AED equipment (training provided).
