POSITION SUMMARY:

Under the supervision of the DON the Wound Care Coordinator adheres to the standards of care, manage the environment to maintain resident/patient safety, and supervise the resident patient care activities. Provide skin care treatments to residents under the medical direction of the resident's attending physician and per the facility's skin care program. This position is dedicated to the promotion of good skin care, the healing of existing skin conditions and the prevention of skin breakdown/injury. Supervise day-to-day C.N.A. compliance with toileting, positioning, incontinence care, assistive/positioning devices and general hygiene care. Ensure/supervise that C.N.A. personnel are completing bathing sheets accurately and reporting skin conditions in a time fashion. Ensure/supervise that the C.N.A.'s are able to provide proper nutritional care to promote healing and good skin care. Provide other skin-related licensed nursing care to residents in accordance with current federal, state and local standards, guidelines and regulations. Monitor and direct as necessary the provision of non-licensed ancillary department services in the absence of ancillary department supervisors (i.e. dietary services, housekeeping services, laundry services, maintenance services, and security services). Complete accurate and timely clinical record documentation per facility policy and State/Federal guidelines. Follows all policies and procedures and performs duties as defined by the State Nurse Practice Act.

REPORTING RELATIONSHIPS:

Oversees all nursing staff as it relates to wound care coordination Wound Care Coordinator

ESSENTIAL FUNCTIONS:

1. Coordination of all pressure ulcers and wound care needs.

2. Following all treatment orders.

3. Report Daily skin care.

4. Assist with all skin care sheets are completed weekly per facility protocol. Evaluate, coordinate and direct appropriate skin care from all levels of nursing services.

5. Assess residents to determine high, medium or low risk for skin breakdown. Develop and assure prevention program is in place. Monitor and direct C.N.A. and nursing staff provision of care to assure proper skin care protocols.

6. Personally evaluate all admits/re-admits within 24 hours.

7. Monitor and ensure that C.N.A.'s and licensed nurses are providing preventative skin care protocols. Provide training, in-servicing and disciplinary actions as warranted and deemed appropriate.

8. Assess all residents with Foley catheters to determine necessity and/or ongoing need as it relates to wound care. In addition, duties include as assigned by DON as needed upon admission:

  • Assess g-tube sites
  • Assess Picc line sites
  • Assess ongoing needs of Foley catheters
  • Changing Foley catheters as ordered by the attending physician as directed by DON. Assisting nurses when PRN changes are needed.
  • Follow up and monitoring for all residents who require suture/staple removal.

9. Prepare and participate in Skin Care QA Committees and weekly skin meetings.

10. Work with skin care consultants.

11. Participate and develop in-services for all staff regarding treatments, skin prevention program, and early detection.

12. Ensure that appropriate documentation/charting is completed as required and in accordance with established policies and procedures.

13. Ensure and/or work with central supply that an adequate supply of floor stock medications, supplies and equipment is on hand to meet the nursing needs of the residents. Report needed items to the ADON and /or DON

14. Ensure the resident environment is as free from accidents and hazards as is possible. Immediately correct/address potential safety risks as prudent and warranted.

15. Participate in C.N.A. nursing staff performance evaluations as directed by ADON and/or DON,

16. Discipline and supervise assigned C.N.A.'s as warranted.

17. Communicate/report information to residents, resident families, physicians, guardians, governmental agencies and facility personnel as required by facility policy and State/Federal guidelines.

18. Ensure compliance with infection control standards. Immediately correct/address identified instances of non-compliance.

19. Oversee nutritional program as assigned by DON

20. Prepare and participate in weekly NARS as directed by DON. Including but not limited to documentation and follow-up with RD.

21. Ensure all components of the nutritional program and as it relates to wound care and healing are in place.

22. Ensure all medication management is in place as it relates to wound care and healing. Including but not limited to wound healing medications and pain management.

23. Ensure that all elements of the facility's Abuse & Neglect policies are maintained and enforced.

24. Strictly follows policy and safeguards computer passwords and computer use authorities. Understands the importance of logging on and off the computer clinical record system and will consistently maintain all computer protocols.

25. Safety

  • Knows and follows facility rules.
  • Demonstrates proper use of equipment. Reports equipment needs or repairs. Wound Care Coordinator LPN J. D.
  • Follows facility-smoking policies.
  • Reports and documents any incidents or accidents of residents, staff or visitors to the appropriate facility personnel.
  • Reports all hazardous conditions/equipment to Supervisor,
  • Performs duties, which may include transportation of residents, as assigned in Facility Disaster Plan.
  • Uses required protective equipment.
  • Follows infection control standards, policies and procedures.

26. Resident Rights I Knows Resident Rights.

  • Helps the residents/patients exercise and/or protect their rights.
  • Reports resident/patient complaints to management.
  • Maintains confidentiality of resident/patient information.

27. HIPAA

  • Follows and adheres to Extendicare's policies and procedures implementing HIPAA requirements for the privacy and security of protected health information.
  • Uses and/or discloses only minimum amount of Protected Health Information necessary to complete assigned tasks. (Applies only if position requires access to PHI under Role Based Access Grid.)
  • Reports all suspected violation of company's HIPAA policies or procedures to Facility Privacy Designee.

MINIMUM QUALIFICATIONS:

Education: Current and unrestricted active R.N./L.P.N. License in the State.

Experience: At least one year of prior Wound Care experience preferred. Prior experience working in a long-term care facility required

Physical Demands:

Primary activities involve walking and talking. Involves sitting and standing. Travel: Up to 10% Working conditions: Primarily inside with well- lighted and well-ventilated areas.

Skills, Knowledge and Abilities:

Ability to relate positively, effectively and appropriately with residents, families, community members, volunteers, government agencies/personnel and other facility staff. Possess special interest in, and a positive attitude about, working with long-term care residents and the elderly. Ability to make independent decisions when circumstances warrant such action. Must be knowledgeable of nursing and medical practices and procedures, as well as laws regulations and guidelines that pertain to long term

care.

Demonstrates basic computer knowledge and ability with an aptitude to learn company applications, Microsoft Office version 1997, 2000 or XP, MS Word, MS Access, MS Excel, E-mail, Outlook, Internet Explorer.

ENVIRONMENTAL AND PHYSICAL REQUIREMENTS:

The responsibilities of this position involve significant physical activities including standing, lifting (up to 60 pounds unassisted), bending, stooping, pushing, pulling and twisting. All employees of nursing homes may be required to provide lifting and transfer assistance to residents. Lifting and/or transferring some residents will require use of a lifting device and /or assistance of other staff.
