Cary Medical Center has an exciting opportunity for a Physical Therapist Assistant to join their Rehabilitation team. The successful candidate will be responsible for the implementation & documentation of treatment plans for patients/clients to relieve pain, restore function, and prevent/reduce disability from pediatric to the geriatric population with the goal of maximizing functional independence.
Duties include but are not limited to:
1. Provide accurate and thorough treatment for patients in a variety of settings. Varieties to include but not limited to the following:
(a) Inpatient, outpatient, homecare, skilled nursing, school, nursing facilities, and contracted services.
3. Per supervising physical therapist, implements appropriate treatment plans aligning goals of patient, family, payor, and caregiver.
4. Deliver highest quality of patient care.
(a) Documentation within 72 hrs for hospital outpatient service.
(b) Payor certification form within 24 hrs of service.
(c) Inpatient hospital documentation within 24 hrs of service.
(d) Documentation for contracted services shall be according to contract standards.
5. Document daily services rendered, including daily progress notes and charges
Requirements
1. Maine State Licensed Physical Therapy Assistant
2. CPR Certified
3. Valid driver's license
Schedule:
With consideration to our patients’ and staff needs, you may need to work on days, evenings, nights, weekends and/or holidays and may need to float to other units.
