JOB SUMMARY
Under the direction of the Community Support Supervisor, the Adult Mental Health Case Manager is responsible for providing adult mental health targeted case management services to individuals experiencing serious and persistent mental illness. Positions assigned to this classification level perform professional social work services involved in the assessment, evaluation, planning and implementation of human service plans to assist participants of the program. This position involves partnering with people to connect them with the services and resources they need to reach their life goals.
The full pay range for this position is $72,113.60 to $108,180.80. Starting pay range is $72,113.60 to $90,147.20 depending on qualifications.
TYPICAL DUTIES AND RESPONSIBILITIES
- Implements and provides professional social/human services to clients, their families and/or the community.
- Implements and performs all case management requirements, documentations and monitoring required to meet mandates, to document services, and to meet the needs of clients.
- Performs a variety of community relations, public relations and outreach services in assigned program areas.
- Collaborates with other professionals, providers, therapists, health professionals, internal departments, law enforcement, community or governmental agencies in coordinating resources and in meeting the needs of clients.
- Performs other comparable duties of a like or similar nature as assigned.
MINIMUM QUALIFICATIONS
All applications will be screened for minimum qualifications, scored and ranked. The highest ranked candidates meeting minimum qualifications will be recommended for the scored oral examinations (interviews). Final selection for the position will include successful completion of oral interview(s), reference checks, and background investigation, including 604 verification and MN DHS NETStudy 2.0.
