Summary

Under the direction of the department manager and supervision of a Charge Nurse, demonstrates proficiency in administering the fundamentals of the nursing process, contributes to the nursing assessment, nursing diagnosis, planning, implementation and evaluation. Develops and participates in the care plan of the patient. Employees in this position will not be primarily assigned to a specific unit, the expectation is to be assigned to a uit when they report to work. Population served by this position are in all age groups.

Responsibilities

Patient Assessment 

  • Obtains and documents nursing history from the patient and/or significant other that is comprehensive of present condition and prior problems. 
  • Performs basic assessment of the patient, indicating adequate knowledge of anatomy, physiology, and pathology. 
  • Assesses vital signs, notes changes, and pursues more specific investigation as needed. 
  • Makes pertinent observation, using language that correctly describes anatomy and physiology and draws logical conclusions which direct formation of nursing care plan. 
  • Identifies abnormal diagnostic data and shows a fundamental understanding of its implications by reporting information to the physician and others caring for the patient, and by incorporating information in the nursing care plan. 
Nursing Care Plan 
  • Uses assessment information to develop a care plan that communicates enough information for incoming personnel to adequately care for the patient. 
  • Reviews and develops care plans for assigned patients. 
  • Includes the patient and his or her family in planning care, establishing priorities, and setting goals. 
Plan Implementation 
  • Uses care plans as a basis for providing safe and therapeutic care to patients. 
  • Consistently follows established standards, policies and procedures in providing nursing care. 
  • Accepts accountability for clinical care of assigned patients, including supervision of non-professional personnel. 
  • Communicates with the patient and his or her family and other members of the health care team in carrying out the goals of the care plan. 
  • Indicates an understanding of the roles of other health care team members in patient care and incorporates their input into the care plan. 
  • Performs competently those technical skills considered basic to the type of care given on that unit. 
  • Identifies patient teaching needs and provides elementary instructions for the patient and his or her family. 
  • Notes discharge planning elements and incorporates these in care plans. 
  • Responds appropriately in urgent and/or emergency situations. 
  • Acts independently and makes decisions under stress as necessary. 
Evaluation 
  • Evaluates and notes the individual patients response to nursing care defined in the nursing care plan and medical records. 
  • Identifies strengths and weaknesses of nursing care systems and actively participates in change by utilizing the Performance Improvement process. 
Leadership 
  • Supervises care given by non-professional personnel, providing instruction as needed. 
  • Regularly attends unit conferences and makes recommendations for improvement in patient care via the patient care conference and unit conference. 
  • Demonstrates customer oriented approach through the expression of respect and understanding when communicating with others. 
  • May be required to function as Team Leader. 
Education: 
  • Maintains professional competence through formal education, workshops and unit specific education. Assists in teaching individual and groups of patients in established programs. 
  • Seeks guidance when encountering nursing procedures and responsibilities that have not been mastered. 
  • Demonstrates proficiency as defined in the unit specific competency standards. 
ADDITIONAL RESPONSIBILITIES:

Qualifications

To perform this job successfully, an individual must be able to perform each essential function satisfactorily. The specifications listed below are representative of the education and experience as well as knowledge, skill and ability required. 

  • Valid Nevada RN license and CPR/BLS required 
  • ACLS within 3 months of employment; completion of Basic Dysrythmia course or examination preferred. 
  • A minimum of one year Acute Care experience required 
  • Must be able to float to Emergency Observation Unit (EDO), Telemetry, Medical/Oncology, Surgical/Ortho, Rehab and Outpatient Procedures 
  • Willingness to float to PEDS preferred 
  • Must be able to Admit/Discharge as needed, and/or care for patients on above listed units 
  • CHEMO, EPIDURAL and/or other certifications preferred.
