FT SOB $5,000 for a 2 Year Commitment 
Full Time / Part Time and PRN positions available

JOB SUMMARY The Physical Therapist is a licensed professional who directs, supervises,evaluates and provides physical therapy services to the patients as prescribedby the attending physician. May supervise Physical Therapist Assistants andAides.

QUALIFICATIONS
Graduate from an accredited physical therapy school. Preferably two years ofclinical experience (if applicable). Capable of lifting the equivalent weightof an adult-size patient. Licensed by the state in which the Physical Therapistresides or practices Physical Therapy according to state requirements.

PHYSICAL DEMANDS Requires moderately heavy physical exertion on a regular and reoccurring basissuch as extensive driving, assisting patients in transfer activities (wheelchairto bed, to tub, to commodes) and providing substantial support to individualsin ambulation therapy. Regularly required to use hands and arms. Frequentlyrequired to stand, walk, reach with hands and arms, stoop, kneel, crouch, orcrawl. Occasionally required to sit. Occasionally lift and/or move over 100pounds. Specific vision abilities include close, peripheral, depth perceptionand ability to adjust focus.

WORKING CONDITIONS
General office, health care setting - exposure to infectious disease.

ESSENTIAL FUNCTIONS
1. Provides treatment as ordered by the attending physician. Interprets, plans,establishes, and exercises proper treatment program in accordance with thephysician’s referral, periodically, reviewing the patient’s program and responseand making recommendations to the physician relative to the need of continuingor discontinuing the physical therapy treatment. Assists the physician inevaluating disability, injury, or disease process of individual patients and inevaluating the patient’s level of function.

2. Contacts the physician directly for patient reports and care as needed.Observes, records, and reports to the physician the patient’s reaction to thetherapy program or any changes in the patient’s condition. Submits to the physicianand the Program Supervisor appropriate written reports to the patient’sprogress at the designated intervals using appropriate and designated forms.

3. Assist in determining the need of the patients an in developing the totalplan of care.

4. Instructs, teaches and supervises patients regarding physical therapyprocedures as related to the patient’s therapy programs.

5. May supervise Physical Therapist Assistant(s) and co-sign PhysicalTherapists Assistant’s progress notes.

6. Adheres to all applicable federal, state, local and company maintainedstandards of care and ethics and policies concerned with the practice ofphysical therapy and among other things, personnel qualifications.

7. Participates in staff development activities and in in-services educationprograms.

8. Demonstrates evidence of continuing education conducted for in-servicesessions.

9. Meets productivity and utilization expectations as established by theCompany and the facility.

10. Completes timely clinical documentation to include functional goal-setting,progress and therapeutic approach.

11. Completes administrative/billing documentation timely and accurately.

12. Covers more than one facility as required by the need of the Company.
