Description

The Intensive in-Home services Qualified Professional will work with a team approach designed to address the identified needs of children and adolescents who, due to serious and chronic symptoms of an emotional, behavioral, or substance use disorder, are unable to remain stable in the community without intensive interventions. The Qualified Professional will advocate for children, adolescents and their families, in order to build on strengths and addresses identified needs. The role will service children and adolescents who, as a result of their mental health or substance use disorder treatment needs, are unable to benefit from participation in academic or vocational services at a developmentally appropriate level in a traditional school or work setting. This medically necessary service directly addresses the individual’s mental health or substance use disorder diagnostic and clinical needs. 

Requirements

Responsibilities & Duties:

• Provide therapeutic activities and interventions for children and adolescents who are at risk for out of home placement
• Participates in the person-centered planning process.
• Provide psycho-education as indicated in the Person Centered Plan.
• Consults with identified medical (such as primary care and psychiatric) and non-medical providers (for example, the county department of social services (DSS), school, the Department of Juvenile Justice and Delinquency Prevention (DJJDP), engages community and natural supports, and includes their input in the person-centered planning process.
• Participate in scheduled Utilization Review Meetings.
• Assist with crisis interventions.
• Assist with implementing a home-based behavioral support plan with the youth and his or her caregivers as indicated in the Person Centered Plan.
• Assist the team leader in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person Centered Plan.
• Work with 2 to 3 team members, led by a therapist, with a maximum caseload of 12 members per team
• Learn and implement our evidence-based practice, Ecosystemic Structural Family Therapy 
• Assist with leading Child and Family Team meetings and developing/uploading Person Centered Plans and other required documents 
• Meeting weekly as a team to staff cases and determine the best plan of action for member

Educational Requirements:

• Bachelor degree in Human Services or related field of study
• A minimum of 2 years experience working with the MH/SA/DD population
• Possess a Qualified Professional credential in the state of North Carolina
