MDS Coordinator

The Well-Being Department provides Assisted Living, Skilled Nursing, primary and acute healthcare, physical therapy, occupational therapy, speech and language pathology, social work services, pastoral counseling, life enrichment and fitness opportunities. The department is seeking a Minimum Data Set (MDS) Coordinator to work with the interdisciplinary team to manage the MDS, short-term rehab referrals and discharge planning for the 30-bed SNF.

RESPONSIBILITIES AND DUTIES:

  • Coordinate Minimum Data Set (MDS) process and complete MDS Nursing Assessments.
  • Maintain knowledge of current MDS requirements and regulations.
  • Develop and maintain comprehensive care plans for all skilled nursing residents
  • Participate in care plan meetings for SNF residents.
  • Educate MDS team on appropriate MDS documentation, processes and requirements.
  • Coordinate interdisciplinary review process for short-term rehab and long term stay referrals and communicate with hospital care managers.
  • Perform nursing discharge planning and orders (FL-2 if needed)

REQUIREMENTS:

Education:

  • Current unencumbered Registered Nurse or Licensed Practical Nurse licensure in the state of NC

  • Current CPR/BLS certification 

  • AANAC Resident Assessment Coordinator Certification (RAC-CT) preferred

Experience:

  • 3-5 years as MDS Coordinator

  • Full Time: Monday-Friday
