Job Overview:
We are seeking a highly skilled and compassionate Registered Nurse Case Manager (Per Diem) to join our team. As a Case Manager, you will be responsible for coordinating and managing the care of patients, ensuring they receive the appropriate medical services and resources. Responsibilities:
- Conduct comprehensive assessments of patients' medical conditions and needs
- Develop individualized care plans in collaboration with the healthcare team
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Monitor and evaluate patients' progress toward achieving health goals
- Provide education and support to patients and their families regarding their medical conditions and treatment plans
- Collaborate with other healthcare professionals to ensure continuity of care
- Assist with discharge planning and coordination of post-discharge services
- Maintain accurate and up-to-date medical records
- Adhere to HIPAA regulations and maintain patient confidentiality
Qualifications:
- Valid Registered Nurse (RN) license in the state of Arizona
- Proven experience in case management or a related field
- Strong knowledge of medical terminology, physiology, and healthcare practices
- Familiarity with managed care principles and insurance requirements
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Strong organizational and time management skills
- Proficient in using electronic medical record systems
