This position is for our Roanoke Hospice team, working at Carilion Roanoke Memorial Hospital as a Liaison Nurse between Hospice and our G eneral Inpatient patients. M-F. Rotating on call is BACK UP ONLY to our staffed triage and primary on-call personnel who is already scheduled.
Seeking individual who feels a professional calling to caring for patients with serious and complex illness at end of life. Position is situated in the beautiful Roanoke Valley. Responsibilities include coordinating, guiding and directing the care of patients facing life limiting illness. Position is primarily during daytime hours, though after hours admissions and visits may be required while in on-call capacity.

What We Require:

Education: Graduate of school of professional nursing.
Experience: One (1) year experience in hospice, home health, oncology, or medical surgical nursing preferred
Licensure, certification, and/or registration: Currently licensed as Registered Nurse in the Commonwealth of Virginia or new graduate of approved program eligible to sit for the Virginia licensure exam. For Home Health, must have valid Virginia Drivers License and pass a Criminal Record Check as per Virginia state law. Must obtain /maintain AHA BLS-HCP.
Other Minimum Qualifications: Must successfully complete orientation and competency requirements. Keyboarding skills required for automated computer systems, including electronic medical records. Must be able to meet essential job functions.
