Job Summary

As part of the health care team, assumes primary accountability for nursing coordination and provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care.
Responsibilities
Assesses, plans, implements, and evaluates patient care in order to achieve optimum patient outcomes for patients of all ages as appropriate to service area.
Determines and performs within clinical, leadership, and/or education pathways to support (direct/indirect) evidenced based, quality patient care service and performance improvement.
Consistently deliver the best in customer service and patient care. As a team member, demonstrate respect, dignity, kindness and empathy in each encounter with patients, families, visitors and other employees.
Education
Bachelors Degree or equivalent in Nursing
Experience
Must successfully complete orientation and competency requirements. Keyboarding skills and computer literacy required for automated computer systems, including electronic medical records. Must be able to meet essential job functions. In Psychiatry and Behavioral Health must possess ability to appropriately handle/restrain patients who may be disruptive and/or physically violent.
Certifications/Licensure/Credentials
ACHPN - Advanced Certified Hospice and Palliative Nurse
