Summary:

Under the administrative supervision of the Site Director who is the designated Case Management Supervisor for that site, the Case Manager I is responsible for the delivery of case management services for a designated caseload of child and adolescent or adult priority clients. The Case Manager I is under the clinical supervision of the Clinical Director or designee.


Examples of Duties:

1. Provides case management services and completes related documentation in compliance with established criteria and time frames for such services.

  • A. Provides home visits to clients, as needed, in order to link, monitor, and assess their overall functioning in their natural environment but in compliance, at a minimum, with the contact requirements of the level of the client’s case management.
  • B. Is responsible for maintaining a designated amount of direct service time with clients and a designated amount of contacts for the assigned caseload.
  • C. Participates in the initial individual program planning process with all service providers (i.e., therapist, medical staff) and provides ongoing information regarding client progress to the service providers.
  • D. Provides assistance to clients in the utilization of community resources in the areas related to independent living skills (i.e., facilitating payment of rent, locating appropriate housing, or purchasing food).
  • E. Provides transportation for clients, when no alternative transportation is available, to appointments for medical, counseling, and other essential services in order to facilitate the client's remaining on outpatient treatment status.
  • F. Completes an individual program plan with each client within 30 days of enrollment into case management.
  • G. Writes progress notes after each service contact with a case management client in accordance with the Center’s procedure on Time Frames for Clinical Records and Related Documentation.
  • H. Maintains documentation of a six month service update in the clinical record.
  • I. Completes a case management assessment prior to the development of the individual program plan.
  • J. Participates in the development of a crisis/safety plan for priority consumers.
  • K. Participates in regularly scheduled Peer Review.

2. Is involved in the initial intervention in crisis situations with case management clients and coordinates the provision of other needed crisis services 24 hour per day, seven days per week, and 365 days per year.
3. Is available as a member of a case management team to case management clients 24 hours per day, seven days per week by means of an on-call cell phone.

4. Participates in weekly group supervision with a focus on client-specific and practice- specific activities on an individual basis with the case management supervisor until the supervisor determines that the case manager’s skill level warrants less intensive supervision.
5. Follows all guidelines outlined in the CAREY Policies and Procedures Manual including Corporate Compliance Policies.
6. Is expected to have regular and predictable attendance and the ability to work cooperatively with others.


Minimum Qualifications
:

Graduation from an accredited college or university with a minimum of a Bachelors degree in a health related field of counseling, psychology or social work, or must be licensed as a nurse (LPN or RN). LPN’s must have fifteen semester hours of behavioral health and one year of behavioral health care experience. Individuals with a Bachelor’s degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen college-level semester hours of course work in behavioral health and at least one year of work experience in the behavioral health setting.
