Job Responsibilities:
Patient education and coordination of care between PCP and other healthcare entities. Will assist with rooming patients, administering vaccines, performing simple in office lab procedures and assist with lab draws as necessary. Assist with health risk assessments, medication refills, notifying patient of lab results, pap log follow up and DME paperwork requirements. LVN will also be trained on how to dispense medications from our in-house class D Pharmacy, assist with inventory and ordering on a monthly basis. Reviewing charts for missing care gaps such as recommended colon screening, mammograms etc. Answers messages and phone calls efficiently and properly documents. Administering vaccines, injections, performing basic dressing changes as ordered by Provider and documents in accordance with Carevide procedures.

Graduated from LVN program. CPR-BLS certification
