Caretruly Healthcare provides home healthcare services to those needing assistance with caring for themselves as a result of old age, sickness, disability and/or other inflictions. Under the direction and reporting to the President, the RN will be responsible for traveling to a patient's home to ensure patient care is administered effectively by their Personal Care Worker (PCW), while the patient maintains their independence. Additionally, the RN will provide clinical knowledge and use their interpersonal skills to answer questions from clients, families, and employees to resolve issues involving nursing care.

Responsibilities

• Identifies client service requirements by establishing personal rapport with potential and actual patients to understand service requirements.
• Assess and chart observations of the patients' home health care at each visit.
• Collaborate with physicians, physical therapy, occupational therapy and all individuals involved in patients' care to devise individualized home health plans for patients.
• Complete evaluation tasks to adhere to all Wisconsin home health care laws and regulations.
• Maintains proper recordkeeping and maintenance of patient records adhering to governmental requirements.
• Coordinates admissions, assessments, and discharges of clients with office staff.
• Provides education to patients and families on proper home health care procedures and strategies.
• Direct supervisor for Personal Care Workers (PCW) aka Caregivers.

Education:

• Graduate of an accredited RN nursing program.
• Current RN license in the State of Wisconsin.

Competencies and Experience:

• Ability to read, write and speak English. Bilingual in English/Spanish a plus.
• 2-3 years of experience preferably in home care services.
• Prior experience with patient care of all age groups.
• Prior leadership experience preferred.
• Flexibility and patience to work in a variety of living environments.
• Exhibits advanced professional knowledge and leadership skills.
• Excellent communication and interpersonal skills.
• Strong organizational and time management skills, and ability to prioritize.
• Strong problem-solving skills and analytical abilities.
• Must have exceptional attention to detail.
• Must be a self-starter and driven.
• Proficient with Microsoft Office and Google products.
• Travel required, must have a valid Wisconsin Driver’s License.

Job Type: Full-time

Pay: $70,000.00 - $95,000.00 per year
