CareSTL Health is Hiring a Community Health Worker (Behavioral Health) Near Saint Louis, MO

CARESTL HEALTH is a Non for Profit, Federally Qualified Health Center (FQHC) serving St. Louis communities and school - based operations.

Position Summary:

Frontline public health workers are responsible for building trusted relationships and partnerships in the community served; focusing on children in foster care, those adopted from foster care, Division of Youth Services, juvenile justice, and their families. In addition, the Community Health Worker effectively educates and links patients, families, communities, and providers to community-based resources by coordinating with area agencies. This position has responsibility for demonstrating the core values of CareSTL Health: Customer Service, Community, Commitment, Compassion, and Competence.

Essential Functions:

  • Complete the PRAPARE to assess social determinants to coordinate and link to resources.
  • Troubleshoot any barriers the patient might have in adhering to care.
  • Engage in the community through outreach to recruit high-risk patients, including the use of Telehealth.
  • Serve as a liaison/link/intermediary between health/social services and facilitate access to medial, dental, and behavioral health services and improve the quality and cultural competence of service delivery.
  • Build individual, family, and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as community education, informal counseling, social support, and focusing on depression screening, common behavioral health conditions in children and adolescents, and adolescent suicide prevention, and Substance Use Disorders.

Education:

  • High school diploma required.
  • Bachelor's Degree in Social Work, Psychology, Public Health Nursing, Health Education, or related field from accredited school preferred.

Requirements:

6 months to a year relevant experience. A valid driver's license, reliable transportation, and insurance. Must have been in Missouri's foster care program, someone who is/was a parent in Missouri's foster care program, has adopted a child from a foster care program, or who has worked for and/or volunteered with a child welfare, child advocacy, juvenile justice, or behavioral health organization.

Additional Information:

  • Clinic hours are Monday - Friday 8 a.m. to 5 p.m.
  • 12 paid holidays per year.
