Job Summary and Qualifications

Summary:

The Health Plan Enrollment and Outreach Specialist acts as a coordinator between external referral sources, Department of Health and Human Services, local county Department of Social Services (DSS), the CarePartners PACE Interdisciplinary Team (IDT), and patients with a focus on program development and overall program census growth.

This position is responsible for providing community outreach to increase awareness of PACE services and generate incoming referrals. This will include, but is not limited to, formal and informal presentations and meetings with potential referral sources to establish relationships, promote collaboration, and provide education about the PACE program. This position will also be responsible for the development of other marketing and growth strategies and assist in building the brand of CarePartners PACE in the community focusing on optimal customer satisfaction. The Health Plan Enrollment and Outreach Specialist serves as a client advocate and resource, working with potential candidates and their caregivers to ensure the efficient and smooth transition of candidates into PACE, as well as managing the assessment and enrollment process. Collaborates with the PACE IDT to enhance processes that maximize efficiency in use of clinical and non-clinical resources pre-enrollment.

What qualifications you will need:

Required Education:Bachelors degree

Preferred Education: Bachelors degree in Social Work, Marketing, Business, Nursing, Health Sciences or related field

Required Experience:Minimum of two years of experience in social work, marketing, or business/sales experience in a healthcare, insurance or similar environment

Preferred Experience:Minimum of two years working in senior healthcare is optimal. History of facilitating Medicaid applications is a plus. Bi-lingual preferred. Minimum 5 years of experience in social work, marketing, or business/sales in a healthcare, insurance or similar environment.
