Halifax Health | Brooks Rehabilitation - Center for Inpatient Rehabilitation, located within Halifax Health – Medical Center of Daytona Beach, specializes in treating stroke, spinal cord injury, brain injury and complex orthopedic conditions. A full spectrum of physical and neuro rehabilitation services are provided.

Responsibilities:

• Completes all IRF PAI assessments as required by CMS.
• Ensures the appropriate personnel have contributed to the IRF PAI assessments.
• Maintains IRF PAI competencies through routine continuing education.
• Monitor the admission and discharge assessments to ensure compliance with CMS regulations.
• Monitor IGC/CMG/co-morbidity for each patient and makes adjustment as needed.
• Maintain clear communication to all departments regarding deadlines for all areas of assessments.
• Complete transmission of the IRF PAI data as stated in Medicare Regulations and CMS Transmittals.
• Performs other duties as assigned by the Manager of Therapy Services/Director of Rehab.
• Verifies IGC and co-morbid conditions on the ADM and D/C IRF PAI assessment to ensure adequate reimbursement for all Medicare patients. 
• Monitors pre-admission assessments, checks nursing and dietary documentation for proper coding of conditions such as morbid obesity and relays the information so codes are added accurately. 
• Monitors updates from CMS and eRehab and communicates information to other departments.
• Collaborates with Coding Coordinator to ensure all codes are captured accurately.
• Reviews all IGC codes and corrects any preliminary preadmission codes when appropriate.
• Provides coding audit results to clinical teams and provides routine education on appropriate coding and GG item scoring.
• Participates in the telehealth processes when appropriate.
• Participates in inter- and intra- institutional programs and committees and/or special projects as assigned.
• Demonstrates knowledge and compliance with department and rehabilitation center policies and procedures. 
• Demonstrates professional behavior in keeping with rehabilitation hospital standards, and professional ethics.
• Acts as a resource person to other departments, team members, physicians, and students

Qualifications:

• Current RN/LPN license in the State of Florida, or PT, PTA, OT, or COTA with a minimum of one year clinical experience
• Must possess strong computer, communication and organizational skills. Must be flexible, detail oriented, able to manage stress, and adept in problem solving
• Maintains confidentiality of patient information.
• Adheres to policies and procedures specific to patient rights.
• Maintains a clean and safe environment; identifies and reports hazards.
• Assumes responsibility for maintaining required continuing education and other necessary credentials.
• Meets all requirements for mandatory in-services.
• Adheres to policies/procedures specific to universal precautions.
• Adheres to policies regarding attendance, conduct, grooming, and dress code.
• Promotes a positive image of the hospital to outside agencies and the public.
• Participates in performance improvement activities as needed.
