Job Summary: The Care Plan Coordinator is responsible for coordinating the development, implementation, and evaluation of individualized care plans for residents in a skilled nursing facility. Working closely with interdisciplinary teams, residents, families, and healthcare providers, the Care Plan Coordinator assesses resident needs, develops comprehensive care plans, monitors progress, and ensures the delivery of person-centered care in accordance with regulatory requirements and best practices.
Key Responsibilities:
- Conduct comprehensive assessments of residents' physical, psychosocial, and functional status, utilizing standardized assessment tools and resident interviews.
- Collaborate with the interdisciplinary team, including nurses, therapists, social workers, and dietitians, to develop individualized care plans based on assessment findings, resident preferences, and goals of care.
- Facilitate care plan meetings with residents, families, and healthcare providers to review assessment data, discuss care goals, and develop realistic care plan objectives and interventions.
- Document care plans, including assessment data, goals, interventions, and outcomes, in accordance with regulatory requirements and facility policies.
- Monitor and evaluate resident progress toward care plan goals, making adjustments as needed in collaboration with the interdisciplinary team.
- Coordinate care plan revisions and updates based on changes in resident condition, treatment goals, or preferences.
- Communicate care plan updates, changes, and recommendations to relevant stakeholders, including nursing staff, therapy teams, and ancillary service providers.
- Serve as a resource for nursing staff, providing guidance and education on care plan implementation, documentation, and regulatory compliance.
- Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives to promote resident-centered care and enhance care plan effectiveness.
- Maintain confidentiality of resident information and ensure compliance with HIPAA regulations and ethical standards.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) license required.
- Minimum of 2 years of clinical nursing experience in a long-term care or skilled nursing facility setting.
- Strong assessment skills and knowledge of geriatric nursing principles, including physical, psychosocial, and cognitive assessment techniques.
- Experience with care planning, documentation, and regulatory compliance in long-term care settings.
- Excellent communication, interpersonal, and collaboration skills, with the ability to engage residents, families, and interdisciplinary team members.
- Proficiency in electronic health records (EHR) and Microsoft Office applications.Detail-oriented with strong organizational and time management skills.
- Ability to prioritize tasks, manage multiple responsibilities, and work independently as well as part of a team.
