Shift Details: Part-time – 20 hours/week

Compensation: $21-$23/hour

Location: Nolte River Place – Anoka, MN

About the job:

The Service Coordinator provides information concerning community resources to residents to assist them in living as independently as possible. They assess and resolve problems that affect the health, safety, or ability of individual residents to live independently and according to the lease agreement.

Essentials:

  • Refer program participants who need case management, including assessment, care planning, referral and advocacy to appropriate agency.
  • Monitor the effectiveness of care and provide day-to-day support to residents.
  • Works with the Specialist II – Resident Relations to assess and resolve problems that affect the health, safety, or ability of individual residents to live independently and according to the lease agreement.
  • Evaluate and adjust services in the buildings as needed to ensure best outcomes for program participants and residents.
  • Market and promote services to individuals within assigned buildings to ensure that residents are aware of available services.
  • Interpret and promote other VOA services and community programs to residents so that they are knowledgeable about resources available to them.
  • Assists residents in building informal support networks with other residents, family, and friends.
  • Arrange for the provision of group services to residents to meet identified needs including educational seminars, and support groups.
  • Maintain files on residents served.
  • Maintain a professional distance in all relationships with residents.
  • Complete and maintain timely, accurate reports so that program information is available and funding guidelines are met.
  • Collaborate with other VOA services and community professionals to ensure that a wide network of resources are available for program participants.
  • Assist residents in planning of regular activities, implements regular activities along with residents and other staff.
  • Monthly Newsletter, Food programs, Seniors programs and Family programs

Required Qualifications: 

  • Bachelors degree in social work, Gerontology, Psychology or Public Health, preferred
    • Associates degree and experience working with seniors may be considered
  • Knowledge of services available in the community for the population being served
  • Training in the aging process, elder services, drug and alcohol abuse, mental health issues
  • Awareness of eligibility for and procedures of federal and state entitlement programs
  • Knowledge of legal liability issues relating to service coordination
  • Experience working with older persons in a direct service capacity is strongly desired
  • Completion of 36 related classroom or seminar training hours prior to hire or within the first twelve months as Services Coordinator
