LPN - HOME HEALTH Visiting Nurse

CareCo Medical is a supportive agency currently recruiting per diem LPN’s to join our Home Health Agency in New London and Windham counties. Our nurses visit a diverse population performing skilled nursing visits, alongside our cross-functional care teams. 

Duties & Responsibilities:

• LPN will provide professional nursing services in accordance with the plan of care established by the Patient Care Coordinator.
• LPN will promote favorable outcomes through collaborative practice patterns and the appropriate and cost-effective use of resources.
• LPN will document accurately, thoroughly, and concisely to demonstrate evidence of appropriately delivered home health services; to facilitate interdisciplinary coordination of services; to justify reimbursement; to decrease the risk of legal liability; to demonstrate compliance with applicable licensure, certification, and accreditation requirements; and to provide a database for quality improvement activities.
• LPN will demonstrate positive and effective interpersonal relationships.
• LPN will demonstrate personal accountability for accomplishing work assignments and for professional growth.
• LPN will educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the patient's health, safety, and independent living.
• LPN will maintain a cooperative relationship among all healthcare teams.

Job Requirements:

• Successful completion of nursing education from an approved school of nursing.
• Current Connecticut LPN licensure
• One (1) year of clinical nursing experience in an acute care or long-term setting, and home health experience preferred but not required.
• Excellent clinical nursing skills.
• Knowledge of accepted standards of medical record keeping and use of EMR system.
• Ability to communicate effectively, orally and in writing.
• Possess high-level problem solving, critical thinking, and reasoning skills for use in patient care planning and problem resolution.
• CPR certified.

Fulfill the role of liaison between patient/family, other health care providers, and our home care cross-functional care team to ensure necessary care is provided promptly and effectively.
